Continuity mistake: Gary (disguised as Sam) sits at the bar and orders a banana daiquiri. The woman next to him tries to engage him in a conversation as he sips his drink, and each time the camera switches between him and the woman, the level of his drink changes noticeably up and down.
Other mistake: When Dean is listening to Sam's voice mails that he left while in Gary's body, the voice in the recordings are of Sam. If Sam was in Gary's body, they would have been of Gary's voice.
The Song Remains the Same - S5-E13
Factual error: When Anna travels back in time, she lands on the hood of a Pontiac trans am. Castiel later comments Anna is in the year 1978, but the the Trans Am Anna falls has the front end of a 1979-1981 model.
Revealing mistake: After Sam (Lucifer) beats the crap out of Dean's face, when he's sitting on the ground, you can tell he's got something stuffed in his mouth to make his cheeks and lip look swollen.
Revealing mistake: When the woman is under the bed with her baby her arm is covered in blood, but when she touches the baby's face and clothes, no blood comes off onto the baby.
Revealing mistake: As Dean digs into the dead guy's guts and discovers the claw the pale dead guy blinks.
Continuity mistake: When Sheriff Mills and Bobby are talking about transferring Rufus, her lapel goes from being under her seatbelt to over several times in subsequent shots.
Mannequin 3: The Reckoning - S6-E14
Continuity mistake: When the guys tip Rose into the grave, her eyes are open, but as she lands and dirt falls over her face, her eyes are closed. A dead person's eyes don't close on their own. (00:25:20)

Audio problem: In 6.14 "Mannequins 3: The Reckoning", when Dean goes up to talk to Ben, Ben is playing a video game. Dean says something along the lines of "There's no way plants could kill zombies", therefore pointing out that Ben is playing Plants vs. Zombies, but the sounds coming from Ben's gaming system are not from Plants vs. Zombies.
Continuity mistake: Season 6 Episode 15 'The French Mistake': When Sam and Dean are first talking to Misha, the collar of his shirt is tucked into his jacket, as normal. However it suddenly is sticking out over the top of his coat, then changes to being tucked in again, with no time for him to have changed it himself.
And Then There Were None - S6-E16
Revealing mistake: Samuel is very much dead, laid out on the table in the canning factory employee lounge, but as Sam stands over him, talking about whether or not it was right to gank him, you can see Samuel's chest rising and falling as he breathes. (00:26:30)
Factual error: In the opening, H.P. Lovecraft is calming his nerves with alcohol. In real life, Lovecraft was a complete teetotaler, who even wrote stories about the evils of liquor.
Revealing mistake: When Dean cuts leviathan-Dean's head off, if you look closely you can see that a) the decapitated head disappears while it is still in the air, and b) when the body recoils, its hands disappear.
Continuity mistake: Elliot Ness and Dean are in his car waiting for the god of time. Elliot takes out his flask, takes a drink, offers some to Dean. Dean declines and Elliot puts the flask back in his pocket. The next shot Elliot is taking a drink again without pulling it back out of his pocket. (00:35:00)
Plot hole: In episodes where Sam and Dean are required to perform spells/rituals at certain times, the issue not quite seen is that not all clocks are exactly in sync with each other, meaning that the clock they are using could be off by a few minutes especially if dealing with different time zones or time periods. (S07E12, Time After Time).
What's Up, Tiger Mommy? - S8-E2
Visible crew/equipment: You can see the crew member's leg go up in the air, and he catches Sam as he runs down the stairs. (00:36:00)
Continuity mistake: In the opening scenes, Vamps drags Chrissy Chambers through the shattered passenger-side window of their crappy vehicle, but by Sam and Dean's arrival, that window is magically unbroken. (00:02:30 - 00:15:55)
Taxi Driver - S8-E19
Continuity mistake: When Crowley comes to the ship and finds Kevin Tran he blew out the glass on the ship to get rid of the sigils. When Sam and Dean get to the ship to check on Kevin, and they notice he's gone, all the glass isn't broken and the sigils are still there.
Continuity mistake: During the scene when Sam and Dean enter the abandoned shipping warehouse to save Charlie, you can see Charlie's legs crossed while sitting in one shot, but in the next shot they are uncrossed. (00:27:10)
Revealing mistake: Dean borrows Charlie's phone to call Sam. On the phone, the lock screen can be seen, showing that he never entered in any number.
Answer: Over the next few episodes, it is revealed that something is not quite right with Sam - that his soul is missing since he returned from Hell, and he does not feel any emotion or attachment to his brother.
