Season 1 generally

Continuity mistake: Season one takes place over five weeks. Janice Parkman goes from not being visibly pregnant to being at least five or six months along during the course of those five weeks.

Don't Look Back - S1-E2

Revealing mistake: Claire Bennett is accidentally tackled when she and Zach are arguing while watching the football game. Apparently, her neck is badly broken in the fall, even twisted a full 180°. However, if you watch closely as she falls, you can see that she is completely fine, stretching out her arms to take the impact before rolling around. She lands on her front, arms stretched out, all in all well prepared for the landing, and rolls around. In the next shot her neck is twisted in a way that simply wouldn't be possible in such a fall. She would have had to have her head stuck under something before rolling over, which she certainly did not. The guy tackling her did not land on top of her either. (00:19:40)

Don't Look Back - S1-E2

Deliberate mistake: The newspaper that shows Petrelli's victory has repeated paragraphs through the article. The article seems to be composed of three paragraphs. One paragraph starts with "I was," and two paragraphs start with "The."

Don't Look Back - S1-E2

Other mistake: When Niki looks at the Cadillac's registration, it says the license plate number is "307RTH." When she opens the trunk, you can see the plate and it's "329OCX."

One Giant Leap - S1-E3

Factual error: During Hiro's journey around New York, a Carl's Jr. restaurant is visible in the background. That restaurant isn't called "Carl's Jr." on the East coast; it's called "Hardee's."

Suggested correction: It's not a dark jacket. It's still his shirt but since he's in the shadows, it just looks darker. You even see when the light swings you can catch a glimpse of the shirt.

Hiros - S1-E5

Continuity mistake: When Peter says that he can finish Isaac's painting himself, Peter raises a paintbrush up to his chin in thought, but in the next shot the brush has disappeared. (00:30:35)

Hiros - S1-E5

Continuity mistake: When the would-be robber takes his gun out, he places his hands on his head. But in the next shot, at least his left hand is no longer on his head. It's back up in the next shot.

Better Halves - S1-E6

Revealing mistake: When Niki goes to find the 2 million in a case and open it, you can tell that all the money is fake as all the $100 bills have the same numbers. (00:37:37)

Seven Minutes to Midnight - S1-E8

Continuity mistake: When Mohinder is back in India, his mother tells him that his sister Shanti died at the age of 5, when Mohinder himself was 2. In "The Hard Part", Mohinder tells Molly Walker that Shanti died before he was born, and shows her a photo of Shanti (who looks 5 years old) and his pregnant mother.

Six Months Ago - S1-E10

Continuity mistake: When Hiro gives Charlie the Japanese phrase book, Charlie flips the book over to look at the back cover, then flips it frontwards again. In the next shot, she's instantly looking at the back cover again. (00:16:20)

Six Months Ago - S1-E10

Continuity mistake: When Charlie invites Hiro over to help celebrate her birthday, the invite is almost immediately after she says "hold that thought" and walks away from his booth. But in that scene in the previous episode, Charlie was already across the room with the other townspeople when she signaled Hiro to come over.

Fallout - S1-E11

Continuity mistake: When Peter is having his vision of New York, D.L. can be seen picking up Micah and running away. When the camera pans out to show Matt holding them back, D.L. is picking up Micah again.

Fallout - S1-E11

Continuity mistake: In previous episodes, when we see DL getting into the car as Nikki prepares to shoot him, he pauses for a second and looks around. In this episode, when we see the actual shooting, DL never looks around, he just opens the car door and prepares to get in, without pause.

Distractions - S1-E14

Continuity mistake: When Peter and Claude are invisible and walking in New York, just after Claude has released his hold on the T-shirt rack, they bump into a man in a coat. Cut to a different angle, showing the two on camera, and the man in the coat is gone. Instead there are two girls and a man with glasses walking directly behind Peter and Claude, none of whom were there a split second ago. (00:02:05)

Distractions - S1-E14

Other mistake: After Peter falls on the cab, a piece of metal is sticking out of his chest. He pulls himself off it, but when he's on the hood, the piece that was sticking though him isn't there, even though it should be visible.

Trivia: The artwork created by character Isaac Mendez is actually done by Tim Sales, a comic book artist who is actually color blind. He does all his work in black and white and has other people insert the color.

Chosen answer: Because he's an idiot. From the story point of view, they needed the vault to actually be physically open so that the release of the virus would be a credible threat. Unfortunately, this required that Peter conveniently forget that he could simply walk through The Wall and use his telekinesis to tear the door open instead. Peter's increasingly large arsenal of abilities, where he ended up having pretty much any ability required for any situation, led to a situation where it became necessary to either have Peter apparently forget that he had a given power, as with the vault door scene, or become naively trusting, as with his refusal to scan Adam's mind to confirm his good intent. This problem, which simply made Peter look increasingly dumb as the show progressed, may well have been a key factor in the decision to severely reduce his ability during season three.
