Continuity mistake: Throughout this episode, West's hair keeps switching from being parted on the left and being flat to being combed from the right in a sweeping higher volume style. Before the title appears, in the inside fight scene, it is parted on the right. In the scene after, the part of West's hair has moved to the left side and remains during the scene in the garden house. When West goes through the garden house trap door, right after he enters the home through a secret passage, West's hair is combed from the right with the sweeping more volume hair. After the fight in that room, as he is breaks through the glass, once outside, his hair goes back to being parted on the left and being flat again. The next scene his hair changes to the more full version. West's hair goes flat again when he is with Gordon in the garden house right before the third frozen panel appears (of the five panels in every episode). His hair changes to right combed version when he enters the home with Gordon through the secret passage and remains this way for the rest of the episode.

Continuity mistake: Before the title appears, there is an inside fight scene in which there is large dirt smudge on the seat of West's right side above his chaps. As he is exits the door, the back of his pants is completely clean.

Visible crew/equipment: As West is swimming in the water tank that is filled with dangerous sea creatures (Hammerhead sharks, etc.) a modern day (1960's) frogman is swimming behind West in the tank.

Continuity mistake: Throughout this episode, the camera shots alternate in showing two trains. After West handcuffs Count Draja on the stage coach, a train is shown with two flags - one on each side of the smokestack (at slanted angles), and the bottom front grill is black. The second time the train appears, the flag is vertical off the side of the train, and the bottom grill has changed to red. The third time the train is shown, there are no flags. The fourth time the train appears, it is the original with two slanted flags and black grill. In the final train shot, the bottom grill has changed to red.
The Night of the Running Death - S3-E15
Continuity mistake: In the last scene in the train's box car, after Princess Silva's departure, all three wine glasses are suddenly almost empty when in the shot before, they were more full. Also, both napkins on the table are rearranged between the two table shots.

Factual error: The soldiers toss Artemus into the cell, slam the door and walk away. But they never produce a key or lock the cell door. He's somehow locked in anyway, though. (00:24:30)

Continuity mistake: In the last scene in the train car before President Grant walks in, two bottles are in the middle of the table (labels facing away from one another) with four glasses behind them. When the President enters and walks towards the table, both bottles are now placed further back (labels are now facing to the front) with the four glass and an ashtray in front of the bottles. After the President refuses a drink of wine, the table is back to the original setup - labels faces away one another and glasses are behind the bottles; the ashtray is no longer there.

Continuity mistake: In the scene before the second freeze frame appears, West is wearing a white shirt with a black neckerchief and blue corduroy jacket. In the close-up of West being handcuffed, he is wearing a blue vest and a different non-corduroy blue jacket. For the rest of the episode, West wears the original white shirt with black neckerchief and blue corduroy jacket.
The Night of the Headless Woman - S3-E17
Continuity mistake: When West comes out of the water, he is soaking wet, but when he sneaks onto the stagecoach and fights with the driver, both his hair and clothes are dry.
The Night of the Vipers - S3-E18
Revealing mistake: The final gun battle in town has a gunman in a tower shooting at West. The gunman's perch is enclosed on all four sides by a bannister, but when West returns fire, shooting the gunman out of the tower, the bannister in front of the gunman has disappeared, allowing the stuntman a clear path to fall out of it.

Revealing mistake: After West shoots the gunman in the tower, the gunman launches himself clear of the tower, rather than merely collapsing and then falling.

Continuity mistake: Midway through this episode as West is ascending the outside wooden steps to Moriarty's office, his shirt has a vertical rip in his upper left chest area. Once West goes inside, that rip is no longer there but is replaced by a much larger vertical rip (much lower from his waist up to his chest). This larger rip is seen throughout this scene. When West leaves Moriarty's office, the larger rip is no longer there and the original smaller higher rip re-appears. In the following scene when West is changing his shirt, the smaller upper rip has been replaced by the much larger lower vertical rip.
The Night of the Vipers - S3-E18
Continuity mistake: As the boxing match moves from inside to outdoors, West's shirt is drenched on the back, as well as around the collar and top front of the shirt. Outdoors, those areas are dry. When the fight moves by the water, West shirt is torn on his front left (length from the 2nd to 4th buttons). At the end of that scene, that tear no longer shows, but a new one appears much higher (length from 1st to 2nd buttons) and is closer to his arm in the upper left chest area.

Continuity mistake: Before the title appears for this episode, in a long screen shot, West is knocked out face-up with both arms extended above his head; only his left elbow is touching the knocked out bad guy next to him. The next immediate close-up shot of West shows West with only his right arm extended above his head and his body is much closer next to the bad guy. As the title appears, West is face-down resting on his right arm when he starts to regain consciousness.

Factual error: The episode opens in New Orleans during Mardi Gras, circa approximately 1873. We hear a jazz band playing "When The Saints Go Marching In" although the song didn't emerge in a recognizable form until the end of the 19th century. (00:00:19)

Factual error: The small round table in West's railcar parlor has a flip-over top, with secret maps hidden on the underside. The wine decanter and glasses all stay attached when it flips. But impossibly, when West turns the tabletop back over at the end, there's a full cup of coffee sitting on it. (00:48:30)

Revealing mistake: After West shoots Major Hazard on the water tower, the falling stuntman lands on a safety cushion that is about a half foot below the ground surface. Further, the next scene cut shows Major Hazard being covered by the tower contents, but Hazard is now lying on the ground surface, not below it like the stuntman.
The Night of the Underground Terror - S3-E19
Continuity mistake: After the third freeze frame, there is an outside fight. West has a major rip in his pants on the left inner seam from the knee to almost his waist - showing his white underwear during the fight. After the camera shot to Gordon and then back to West, his pants are now miraculously mended in the front and inner left leg area with a new large rip on the pant outer right seam from the knee to upper thigh.

Continuity mistake: Towards the end of this episode, West crashes through a window and tears his right upper sleeve directly below the shoulder. When he exits the door into the cave and meets Gordon, the rip is no longer there.
