Continuity mistake: During the time the Lieutenant is discussing Halvorsen's fate with Colonel Barbossa, his left side is facing the Colonel. An instant later when he salutes the Colonel, the lieutenant is now facing the Colonel directly.

The Night Dr. Loveless Died - S3-E4

Visible crew/equipment: The arm of the actual carriage driver is visible poking through the back of the carriage, slapping the horse and signaling it when to stop/proceed with a second set of reins, showing the actor in the carriage was not controlling the horse at all.

Continuity mistake: Eatlier, West smashes the window and removes the drape so he and Liebnicht can escape. Later, when Artie and Triste arrive at the same window, the glass and the drape are not gone, but intact as if West never caused the damage.

The Night of the Infernal Machine - S2-E14

Trivia: West collides with the cook and sends him sprawling face-down in a huge platter of gelatin. Before racing off again, West quips, "Sorry about that, chef!" This was a winking homage to the 60s spy comedy Get Smart and its bumbling hero Maxwell Smart's famous catch-phrase, "Sorry about that, Chief!" (00:47:30)
