Continuity mistake: When Maddie turns the photo album round, its position changes between shots. In the first shot she is holding it by the top of the album, as shown with a brown photo being away from her hands. Camera changes and now she is holding it by the bottom of the album and her hand is on the brown photo. (00:09:40)

Continuity mistake: The pill bottle the agents recovered from the house has the Doctor's name "Fleming" to search for. But when McGee searches the NCIS system, he spells the name "Flemming" and gets his results. As well, the scrip sheets for Dr. Flemming archived on the computer, contain several identification numbers - DEA (AJ0000000000), Batch (00000001) & License (C00000.) - not real numbers - so the prop master didn't even try to fake any of them. Continuity mistake: When the CO opens the drawer of Michael's personal effects, the items change place or change entirely between shots. For example a book appears. (00:13:05)

Continuity mistake: When Abby tells McGee that the suspect has a gun, there is an overhead shot of the top of the building and the brown car is at least 4 spaces away from it. When Abby is watching the report on TV and says "damn you Gibbs", the car is next to the building. (00:04:00 - 00:05:00)
Visible crew/equipment: When Ducky comes in explaining how the victim has one more thing to say, a red tape marker is visible on the floor. (00:37:05)
Visible crew/equipment: Just as the blonde asks if she's needed to ID another photo, there is an overhead shot that includes the camera trolley wheel. (00:35:20)
Visible crew/equipment: When Tony is pacing after they find out Kamal's identity, there is a green tape marker visible on the floor. (00:32:00)
Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
