The Doctor accidentally rips off the handle closest to him and the viewscreen. A minute later when Amy is alone and we are looking at the Angel video, this handle is back up. It's gone again in subsequent shots.
The Vampires of Venice - S5-E6
Revealing mistake: When the Doctor begins climbing a pillar on top of the bell tower, we can see white sky on the central arch. Two other arches in the very same shot show really dark CG sky. (00:42:17)

Continuity mistake: When Amy puts on her poncho and says "If we're all gonna die, let's die looking like a Peruvian folk band", the slit changes orientation between shots as she walks to stand between the Doctor and Rory; from across her shoulders to pointing downwards. Revealing mistake: When the Doctor's jacket is being held by a Weeping Angel while he's monologuing about why they haven't killed him yet, he moves at one point and the Angel's hand moves with him, despite the fact that they're normally effectively stone in situations like this. (00:12:16)

Continuity mistake: When the Doctor is looking through a hole in one of the pages in the book on the Weeping Angels and River is asking him how early in his timeline this is, which of his hands is on the page changes between shots. (00:17:26)
Chosen answer: 1) When Stephen Moffat took over he ignored a lot of what had been developed before (there is not in-universe answer). 2) It would have killed Rose, so the Doctor absorbed the energy. His body regenerated before the energy could do a significant amount of damage that would prevent regeneration.
