Continuity mistake: When Kirk gives the order to Scott for transport, Spock lies in a diagonal position with the yellow spores on his chest with McCoy kneeling by him. After the cut to Scott on the bridge, Spock is lying with his head towards the camera, his chest is clean, and McCoy standing about six feet to Kirk's right. All much too quickly to be normal.
Continuity mistake: As Kirk and party dematerialize, Kirk's arms are by his side. As they are briefly seen in the Transporter room, his left hand is up with the communicator.

Continuity mistake: Uhura's image on the conference viewer is of a close up on her face. When the camera looks at the screen, the image is of her head and upper torso. Likewise, when Fitzpatrick is on, first it's his head and torso, then it's only his head and shoulders.
Continuity mistake: After Kirk knocks the jailer out, there's a bit of stew on the cell's door above the lock. After Kirk drags the man into the cell and closes the door, the bar above the lock is clean and dry.
Continuity mistake: In the first scene when Spock is piloting the shuttle craft out of the Enterprise and into space, as the bay doors open you can see both the ship's shuttles, the Galileo, and right behind it the Columbus. Yet as soon as the Galileo clears the bay doors, in the immediate reverse shot, the bay is empty and the Columbus has disappeared. (00:01:45)

Continuity mistake: As the Galileo re-enters the atmosphere, you can see Scotty in the copilot chair. His hair is parted on the left side, but in the interior shot, it's parted on the right.

Continuity mistake: The cube holding the ship in place is spinning counter clockwise, from left to right, on its axis. When the camera is looking at the bridge, the colors are going from left to right, rather than right to left to match the cube's movement.
Answer: Kirk was getting his physical and Dr. McCoy probably turned off communications, because if he hadn't, Kirk would have left and headed straight for the bridge, leaving McCoy irritated.
