24 Hours (1) (a.k.a. The Longest Day (1)) - S1-E1
Visible crew/equipment: In the last scene there are three doctors standing in the corridor (I think it's Ross, Carter and Benton). You can see their tape marks for where they need to stand. When the camera pans out it pans too far and you can see where the corridor floor ends and the studio floor starts.
24 Hours (2) (a.k.a. The Longest Day (2)) - S1-E2
Visible crew/equipment: When Dr. Greene goes into the lab with Mrs. Raskin, the boom mic is reflected in the glass behind them.
Chicago Heat - S1-E7
Visible crew/equipment: After Dr. Benton talks to the police detective and then goes to see the grocery store owner, as Dr. Benton swings open the doors, the camera crew can be seen in the reflection of the glass of the door.
Visible crew/equipment: When Dr. Greene says "It's my daughter's birthday too" a boom mic is visible.
Visible crew/equipment: At the very start, the camera tracks past the ambulance entrance towards admitting, and when the camera turns as Rolando asks, "What's your ETA?" the reflection of the camera operator is visible on the glass. (00:00:20)
Visible crew/equipment: When Peter examines the little boy again and tells Jeanie and Susan to draw pre-op labs, just as Peter exits the room behind Jeanie, the reflection of the moving boom pole/mic is visible on the door's glass. (00:18:50)
Visible crew/equipment: When Mark and Doug are talking about Jen, Doug says, "If you can't make a marriage work, how the hell am I gonna," and just as Mark joins Susan at the desk, the reflection of the moving boom pole/mic is visible on the glass behind Mark. (00:22:15)
Visible crew/equipment: When the ER team is helping Victor's children and wife you can see a camera's reflection on Lydia's back when she is helping Victor's daughter.
Visible crew/equipment: Lucy is shown getting a coat from under the desk. Yosh comes up to her and says something, then opens the door and walks out of the shot. When he opens the door, there is a reflection of a man wearing headphones and holding a boom mic.
Visible crew/equipment: When the pedestrian Michael hit goes into the trauma room, when the ER doctor closes the door for a brief second the camera is visible.
Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all. Visible crew/equipment: When Carol and Luka follow Robbie to the trauma room with his mother's body they both stop at the doorway to watch Robbie, then just as the camera moves from behind Carol and Luka to in-between them zooming in on Robbie, the reflection of the moving boom pole and mic is visible on the glass behind Robbie. Visible crew/equipment: When Dr. DeRaad asks Kerry if she knows Paul Sobriki she tells him to check the board, and as Kerry walks past the door we can see the reflection of a crouched crew member walking backwards, pulling a rolling microphone stand. (00:02:45)
Visible crew/equipment: When Luka and Mark are questioning Amira about the knife Sobriki had used, Kerry walks in then tells Amira to go upstairs to get security to open the personnel office, and the green T-mark taped to the floor is visible near Luka's feet. (00:16:45)

Chaos Theory (a.k.a. Unknown Origin) - S9-E1
Visible crew/equipment: While Gallant is running on the sixth floor the boom mic is visible in the window.
Answer: A man named Paul Sobriki comes into the ER in the season 6 episode 13 " All in the Family." He is complaining of headaches, Carter and Lucy decide to rule out meningitis and do a lumber puncture. Paul struggles during the procedure and believes that both doctors are stabbing him. Lucy, who is interested in Psych thinks something is wrong with their patient mentally, but Carter is dismissive of her. She gets cross with his attitude and she calls the psych department anyway to have a look at him. Later on and near the closing 20 minutes or so of the episode, all the staff start playing loud music and get a cake which appears to be blue lol, together to party as it's Valentines Day, Carter returns from somewhere and asks where Lucy is. He goes to check one of the rooms for her and Paul Sobriki comes up behind him in the dark and stabs him twice in the lower back, he collapses and as he falls unconscious he sees Lucy on the ground also. In the next episode, Be Still My Heart (ep 14), Dr Weaver discovers them and all of the staff work to try and save them, whilst Carter survives. Lucy's wounds are fatal and suffers a pulmonary embolism as Corday and Romano try to save her. Carter is grief stricken and feels guilty about the way he treated her, he becomes addicted to painkillers and Benton, Greene all help him and book him into rehab.
