Ground Zero - S4-E6

Audio problem: When Greene is outside by the river explaining what happened to him, there are a couple of really bad sound edits in the background noise. These are really obvious as the sound suddenly changes, and for some reason the background noise in this scene is very loud, almost as loud as the dialogue; very strange.

Time of Death - S11-E6

Audio problem: When the team is moving Mr. Metcalf to a different room, Luca says, "Let's go people. Prime the rapid infuser." Abby says, "subclavian on the right." The shot shows the a close up of the patient's face (Mr. Metcalf). Immediately, that 4-second shot with audio repeats itself, before moving on to the next. (00:12:40)

Be Still My Heart - S6-E13

Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all.
