Star Trek: Deep Space Nine

The Ship - S5-E2

Other mistake: Dax says that T'Lor (the blue-shirted alien) had the away team's only medkit, but when he was shot and killed, T'Lor only had a phaser rifle on him.

Suggested correction: Just as T'Lor is shot and falls off the runabout you can see the med kit hanging from his right shoulder on a strap.

The Ship - S5-E2

Character mistake: At the end, Sisko blames the deaths of the runabout crew (Hoya, Rooney and Bertram) on himself and the Vorta not trusting each other. However, the runabout was destroyed before any negotiations had taken place, and even before any Jem'Hadar or Vorta had landed on the planet, so their deaths were largely unavoidable. Even the later deaths of T'Lor and Muniz were caused by the first firefight, again, before any negotiations had happened.

Suggested correction: Alternatively, Sisko could also be referring to the general mistrust that exists between Starfleet and the Dominion, which is constantly threatening to boil over into full scale war. Without that prejudicial mistrust and paranoia, The Jem'Hadar would have likely never killed Muniz or attacked the runabout. Indeed, it can be said both sides inability to negotiate throughout the episode is due to their mutual belief that the other side cannot be trusted under any circumstances.

Trials and Tribble-ations - S5-E6

Continuity mistake: On the old Enterprise bridge, Sisko is shown at a bridge station, and the person next to him, close to Dax's position, is wearing a gold shirt. When Kirk comes onto the bridge, the person next to Dax now has a red shirt, which was the original crewman's shirt in TOS.

The Begotten - S5-E12

Visible crew/equipment: After Odo orders two glasses of champagne for Dr. Mora and himself, when it cuts to the birthing ceremony as Y'Pora walks towards Nerys, there are three T-marks (one blue and two green) taped to the floor, at the bottom right corner of the screen. (00:33:10)

By Inferno's Light - S5-E15

Continuity mistake: When Bashir goes to let Garak out of the crawlspace, he pulls the prying tool from under the end of the bunk mattress closest to him. But in the scene before a Romulan prisoner had put it under the opposite end of the mattress.

Doctor Bashir, I Presume? - S5-E16

Character mistake: In the scene in Sisko's office where he has contacted Rear Admiral Bennett, Bennett says "Two hundred years ago, we tried to improve the species through DNA resequencing, and what did we get for our trouble? The Eugenics Wars." This puts the wars in the 22nd century, and differs from every other mention of them, where they occurred in the late 20th century, not the 22nd century as Bennett implies. A Simple Investigation - S5-E17

Continuity mistake: When Arissa is first interviewed in Odo's office, she displays a 'data port' that is embedded below and behind her left ear in her upper neck. Her hair is styled to make it visible, and the port seems to be there for the entire scene, tho almost completely obscured at times. The second time she's in Odo's office, her hair style is the same, making it very obvious the port has now vanished, and it's gone the entire scene. For the third visit to Odo's office, the port appears and disappears at random. Apparently this scene was comprised of shots from the two separate shoots that made up the first two office scenes. Ferengi Love Songs - S5-E20

Visible crew/equipment: Just after Leeta and Rom makeup and decide to proceed with their wedding plans, the camera pans up to the Chief and Julian. You can briefly see green tape on the floor indicating where the Chief is supposed to stand.

Children of Time - S5-E22

Continuity mistake: The episode includes Odo as one of the people who is still alive after "200 years." However, in later seasons, it is revealed that Odo was infected with the morphogenetic virus by Section 31 quite a while ago. And given that in those later seasons, he is deteriorating and dying, there is no possible way that the "Future Odo" from this episode could've survived 200 years since he likely wouldn't have survived more than a decade since this episode if they became trapped back in time.

In The Cards - S5-E25

Visible crew/equipment: After Kai Winn grasps Sisko's ear to feel his pagh, it soon cuts to Jake delivering half the items to Dr. Giger, and when Giger removes the chip that Jake brought on the cart, the actor's yellow tape mark is visible on the floor near Giger's consoles. (00:31:00)

If Wishes were Horses - S1-E16

Trivia: Buck Bokai's baseball card, a collectible featured on Benjamin Sisko's desk, had actor Keone Young on the front, in character, but showed "Trek" model maker Gregory Jein, who invented the "history" of the character, on the back. The pair bore an uncanny resemblance to each other.

Question: I've noticed in some episodes the person playing Dr. Bashir is credited as Siddig El Fadil and in others he's Alexander Siddig. At what point in the series did he have the name change?

Answer: He changed his name after the third season because people were having difficulty pronouncing his last name."Alexander Siddig" is the name he typically goes by as his full birth name, "Siddig El Tahir El Fadil El Siddig Abderahman Mohammed Ahmed Abdel Karim El Mahdi", is far too verbose and complex for screen credits.
