Revealing mistake: During the fight between Garak and Worf near the end of the episode, a Jefferies tube door is knocked out of its frame to reveal the end of the set - a dark room with a wooden scaffold and various other pieces of equipment (slow-mo helpful but not required). (00:36:00)

Continuity mistake: At the end of the episode, Odo has sent everyone affected by the telepathic matrix to a cargo bay. Sisko and O'Brien remove their com badges, throwing them aside before heading to the bay. After everyone is cured, Sisko straightens back up and, in the closeup, you can see that he has a badge on. In the next closeup, it's gone again.

A Man Alone - S1-E4
Other mistake: Aron Eisenberg's name is misspelled on-screen as Aron Eisenerg. (00:04:25)

Visible crew/equipment: As Chief O'Brien closes the floor panel in the runabout, a mic pole and chord are visible on the left side of the screen behind Sisko. (00:10:35)

Visible crew/equipment: Just after Leeta and Rom makeup and decide to proceed with their wedding plans, the camera pans up to the Chief and Julian. You can briefly see green tape on the floor indicating where the Chief is supposed to stand.

Visible crew/equipment: As the Defiant tries to contact the Ulysses, the camera pulls too far back at Odo's station, and we can see the microphone along with the boom pole above the captain's chair. (00:14:40)

Visible crew/equipment: After Kira returns the expensive gift to Zek, when Quark kneels in front of Zek we can see the T-mark on the floor, under the table. (00:23:15)

Continuity mistake: When Kira is explaining to Jadzia in Quark's that she wants to get rid of Sisko, she is wearing her Bajoran earring in her right ear. When she picks Quark up by his collar a couple of shots later, the earring is gone.

Visible crew/equipment: When Jadzia and Melora are in the runabout talking about relationships, Jadzia mentions having to go back 150 years for a Starfleet relationship; when it cuts to Melora the boom mic can be seen coming in through the front window. (00:23:05)

Visible crew/equipment: After the infant Changeling gives Odo the bittersweet gift, Odo exits the infirmary and shapeshifts into the Tarkalean Hawk. As Odo soars through the Promenade's top level, we can see a director's chair beside the spiral staircase, down below. (00:41:15)

The Way of the Warrior (1) - S4-E1
Visible crew/equipment: At the start, after the Changeling surprise drill's staff is dismissed, while Sisko and Nerys are walking away, when she asks, "How about some dinner?" at the bottom left side of the screen, there are multiple T-marks visible on the floor behind her, ending with a pink T-mark as Sisko exits. A Simple Investigation - S5-E17
Visible crew/equipment: In the cargo bay, Odo shares with Kira the embarrassing "bedroom eyes" comment he had received from Arissa, and when Kira asks, "Are you going to see her again?," we can see the reflection of the camera operator, who is standing directly behind Odo. (00:08:05)

Visible crew/equipment: After Joran supposes that he'll have to make himself more endearing, he suggests to Ezri that she visit the two victims' quarters to find answers. At the start of the next shot, the reflection of the boom operator holding up the boom pole is visible on the glass of the framed photo. (00:29:45)

In the Pale Moonlight - S6-E19
Visible crew/equipment: After Jadzia plays devil's advocate with Sisko, he tells her she would've made a decent Romulan. Then, when Sisko passes the camera as he walks around his desk, the blue T-mark is visible on the floor behind Jadzia. (00:06:10)

Visible crew/equipment: After Odo orders two glasses of champagne for Dr. Mora and himself, when it cuts to the birthing ceremony as Y'Pora walks towards Nerys, there are three T-marks (one blue and two green) taped to the floor, at the bottom right corner of the screen. (00:33:10)

Visible crew/equipment: When Jadzia walks into Kor's quarters and finds Kor on the floor, just as she helps Kor stand up, there's a visible T-mark taped to the floor beside the chair. (00:08:35)

Resurrection - S6-E8
Visible crew/equipment: When Bareil enters his quarters and finds Intendant Kira dressed like Major Kira, just as Intendant Kira walks towards Bareil, in the mirror we can see the reflection of a crew member wearing a white shirt and shorts. (00:36:55)

Visible crew/equipment: When everyone returns to DS9, Worf and Jadzia reunite and kiss. Then, in the next shot, just as O'Brien calls out to Quark, there's a visible T-mark on the floor. (00:42:15)

Visible crew/equipment: After Kai Winn grasps Sisko's ear to feel his pagh, it soon cuts to Jake delivering half the items to Dr. Giger, and when Giger removes the chip that Jake brought on the cart, the actor's yellow tape mark is visible on the floor near Giger's consoles. (00:31:00)

Visible crew/equipment: During the last scene in the infirmary, Bashir has a word with O'Brien about Garak's condition. Then, just as O'Brien walks towards Garak, on the console's reflective surface, we can see the reflection of the moving boom mic (beside the ceiling's black stripe pattern) at the bottom of the screen. Answer: He changed his name after the third season because people were having difficulty pronouncing his last name."Alexander Siddig" is the name he typically goes by as his full birth name, "Siddig El Tahir El Fadil El Siddig Abderahman Mohammed Ahmed Abdel Karim El Mahdi", is far too verbose and complex for screen credits.
