Never Get Out of the Boat - S1-E16
Factual error: Seals arrange their weapons and gear according to the mission they are deployed on, CQB requires short compact weapons, so they'll go with H&K 416 with a 10.4" barrel or the H&K MP7, for open terrain missions they'll swap out their uppers in the H&K 416's and swap in the 14.5" barrels. Bravo team is on a jungle op where no CQB is expected however most of the guys are rolling with 10.4." Clay is the only one with the long distance set up. Sonny doesn't even have his MK46 SAW. Deliberate mistake: The Seal team is dispatched to an uninhabited island in the South China sea called Panatag Shoal. In the operations room the team is briefed that there is dense jungle. When the team gets there they're walking on paths laden with dry palm trees and paths 15ft wide. Dense jungle means you're walking through thick brush and using a machete to cut it. In reality Panatag Shoal has very little vegetation - it's a natural submerged bank which is covered by sand and rises from the bed of a body of water. (00:30:20 - 00:31:27)

Factual error: Clay is in a van, and he's looking at some men possibly standing guard. He uses his Eotech EXPS optic to zoom in on them. An Eotech optic is a zero power magnification optic, so looking through it is like looking through nothing. The only way it can be magnified is by adding a G33 3x or the newer G45 5x magnifier to the system. These magnifiers flip 90° and lock in place when magnification is needed or flip back when its not. Clay's rifle does not have a magnifier. (00:14:00 - 00:15:00)
Containment - S1-E11
Deliberate mistake: Sunny is the teams SAW (squad automatic weapon) gunner. He carries an FN MK46 which is a special operations version of the M249 SAW. The MK46 unlike the M249 only takes belt-fed ammunition from 100 or 200 round "nutsacks." On this mission Sunny is wearing a chest rig with STANAG mags and no nutsacks. He has no backpack for extra ammo either. He hands off his MK46 to the Jason whose staying behind and no nutsacks. Plus he doesn't take Jay's HK416 rifle in the swap. Usually people swap one for one. Ghosts of Christmas Future - S1-E4
Continuity mistake: Jason and his team have AKS-74U's in hand as they hit the target vehicles. Once their target escapes they start giving chase and their weapons disappear from their hands and as they're running no holster are visible with sidearms. Then once they reach the inside of a building they all have pistols in hand. Even if the sidearms were "hollywood tucked" into their pants they would've fallen out during their chase of the target. Other mistake: Ray and Clay are utilizing sniper rifles to combat the hostile enemy. Their shots are both shown through a thermal optic view, however both of their rifles only have standard day optics. For a thermal view to be operational a separate thermal optic needs to be clipped on to the front of the day scope/optic to provide a thermal view. A day scope/optic doesn't magically become thermal. They mount onto the top rail of the Picatinny Rail. (00:29:00 - 00:29:40)
Continuity mistake: Clay usually carries the H&K416 with the 14.5" barrel with the addition of a Surefire suppressor and a variable optic scope, whereas the rest of the team caries the more compact 10.4" barrel with Surefire suppressors and Eotech optics. Sometimes Clay carries the M110 Sniper, all their gear changes up depending on mission. On this mission Clay is with the 14.5" with scope and later when he exits the sub tube he hands the sub crewman a 10.4" with Eoetch. They didn't switch weapons underwater. (00:00:45 - 00:06:00)
Fog of War - S3-E13
Visible crew/equipment: When Trent is approaching the door you can see a GoPro attached to the barrel of his rifle, on the left side of the weapon. (00:08:13)
Other mistake: Jason Hayes is arrested by a NIS (Naval Investigative Service) agent and placed in handcuffs. Still in handcuffs, Jason is walked into the United States Naval Consolidated Brig by Shore Patrol. The handcuffs are removed and Jason empties his pockets. Jason places a knife in a bowl. There's no way a prisoner would be arrested or allowed into a jail or brig without being searched, and there is no reaction to the fact there is a weapon and a prisoner had access to it. (00:01:01)

Character mistake: When Jason is having his toast in the bar, he begins listing up the amount of decorations there are in the room, starting with saying there are three Silver Star medals in the room. Bravo Team alone has eight Silver Star medals. The only operators from Bravo who do not have the Silver Star medal are Blackburn and Full Metal. Full Metal was a real SEAL before going into acting, and earned a Navy Cross that he also wears in the show on his dress uniform. (00:36:36 - 00:37:12)
Character mistake: At the start of the episode when Clay is done brushing his teeth and standing in front of the mirror without a shirt, he has no tattoos. He has been seen several times with tattoos on his arms that are not even covered by a normal T-shirt. (00:05:02 - 00:05:23)
All Along the Watchtower: Part 2 - S3-E6
Visible crew/equipment: When they walk through the hallway towards the second building, a crew member can be briefly seen.
Revealing mistake: Supposedly in Liberia, the team takeoff in a helicopter. However, when they take off, the camera reveals the airfield, where several general aviation aircraft are parked. This is highly unlikely to be in Liberia, also the night lights seem to be somewhere in Cali (perhaps LA)). (00:27:20)
Other mistake: When Clay is engaging a terrorist armored vehicle, his rifle is on safe, he fires a few rounds, then places his rifle on fire, firing a few more rounds.
Factual error: A US Navy Lieutenant Commander (Chaplain) is shown wearing a metal chaplain badge on his working uniform. This is not authorized. Working uniforms require cloth badges. (00:09:40)
The Worst of Conditions - S2-E3
Visible crew/equipment: As the team drives through Mumbai heading down the tunnel you can see the camera man for a brief moment behind the gunner atop the truck.
Unbecoming an Officer - S3-E10
Continuity mistake: Jason is in bed with his girlfriend lying on opposite ends of the bed. The girlfriend's feet are exposed. When the camera pans up to her face and back down, she only has one foot exposed. (00:02:00)
Continuity mistake: A taxi is pursuing a Mercedes sedan. At one point they collide - taxi left side and Mercedes right side. Later in the chase, the scrapes and tire marks on the Mercedes are on its left side instead of its right. (00:08:37 - 00:09:39)
Other mistake: Sonny discovers Lisa's training manual, and it says "Officer Candidates School." It should be "Officer Candidate School." It even says so on the seal right there on the same cover. (00:11:20)
Character mistake: The team is using names of cities in Puerto Rico to show progress on their mission. Condado and Arecibo are misspelled. (00:04:00)
