The Phoenix Saga (1): Sacrifice - S3-E3
Trivia: The orange space suits that the X-Men wear more closely resemble those worn by Russian cosmonauts than the black and white space suits worn by American astronauts. This is likely the result of the animation company in the Philippines being given notes to draw the characters in space suits, but in their research probably came across the Russian design, and not knowing any better or investigating further, went with that style.

No Mutant Is an Island - S5-E4
Character mistake: When Cyclops demonstrates his power to Rusty to show him that he's also a mutant, he shoots an optic blast at a building and blows a gigantic hole in its facade. While the building had the appearance of being condemned, Cyclops had no way of knowing whether there were squatters inside. He could have seriously injured or killed several people for all he knew. Continuity mistake: When the Shi'ar beam down in front of the X-Men, Lilandra and Gladiator have a gap of a few feet between them. When the shot changes, they are suddenly standing closer together. (00:19:14)

Continuity mistake: When Sebastian Shaw is escorting Jean out of the room, there is a chair with a side table in the foreground. A couple of shots later when Wyngarde is sitting in the chair, the side table has disappeared. (00:06:09)

Continuity mistake: When Wyngarde first invades Jean's dream and places her down on the bed, she has a mole near her left nostril. Later in the scene when she is looking overboard, the mole is next to the corner of her lips, and even briefly disappears from her face. (00:13:35 - 00:14:23)
