Continuity mistake: Captain Willard sits in the sunshine near the back of the boat reading his file on Colonel Kurtz but in the wide shots of the boat as Lance is water skiing he's not there. (01:15:20)

Continuity mistake: Captain Willard says, "the army tried one last time to bring him back into the fold" and we see a close-up of General Corman again sitting at lunch with the captain from earlier in the movie. This was not shot at the same time because the general's shirt is different. Look closely at his name, the collar covers everything except the first two letters whereas earlier in the movie, for the entire scene his name was showing and could be read. (01:18:10)

Continuity mistake: On the boat Chief Phillips says to Captain Willard, "my orders say I'm not supposed to know where I'm taking this boat, so I don't". If you look at the binoculars next to Chief Phillips on the right of the screen they rotate 90 degrees between shots. (01:19:40)

Continuity mistake: As they leave the MedEvac we see an overhead shot of the boat and the flag has changed. The back of the flag has been cut up considerably since when they arrived at the MedEvac even though they haven't been it any battles. (01:32:55)

Continuity mistake: As they approach the Do Lung Bridge Chef talks to Lance with his gun muzzle pointing out in front of him. The muzzle isn't near anything but in the next shot it's resting on the flat piece of metal on top of the machine gun. (01:40:35)

Continuity mistake: Chef asks his playmate, Miss May, to put on a black wig as they sit in the helicopter. When he first reaches for the wig it's on top of a Styrofoam head but in the next shot he reaches for it again and it's on a small table to the right of the Styrofoam head. (01:28:50)

Continuity mistake: When Captain Willard shoots the injured Vietnamese woman on the sampan, the T-shirts that we saw before draped over the metal bar in front of Chief Phillips are no longer there even though no one goes near the spot. (01:38:30)

Continuity mistake: As Chef looks over the papers from the sampan crew the woman is standing under the canopy with the sun shining on her. The next time we see her several seconds later she's in the same spot but the sun is no longer shining on her. (01:35:35)

Continuity mistake: While sitting in the PBR at Kurtz's compound, Chef and Captain Willard discuss what they should do as the Captain puts on a clean shirt. Chef says to him, "He's wacko man. He's worse than crazy, he's evil." The background (box behind captain's head) changes. (02:34:40)

Audio problem: Approaching Kurtz' compound Lance starts drumming on two tin cans and if you listen closely they get the overdub wrong. At first we hear two distinct pitches, which are the actual sounds, picked up by the microphone but the last time he hits it we hear a third higher pitch which was overdubbed. There are only two cans not three and he doesn't change the way he's holding them therefore you can only have a maximum of two audible pitches. (02:29:05)

Continuity mistake: At Kurtz's compound Lance sits at the front of the PBR watching them chop down the dead body hanging from the tree. The machine gun behind him rotates 45 degrees counter clockwise between shots. (02:34:30)

Continuity mistake: As they sit in the boat at Kurtz's compound discussing what they should do, Captain Willard tells Chef about the air strike and hands the radio to him with the antenna standing straight up. In the next shot as he grabs it the antenna is folded over. (02:35:45)

Revealing mistake: A Medevac helicopter lands in the courtyard and later explodes when a Vietnamese woman throws a grenade into it. The copter that lands is not the one that explodes. The front of the helicopter that blows up is older and beat up and the window on the pilot's door does not have any green sun shade on top as the other chopper had when it landed. The one that explodes is also on a platform that collapses as it blows up. (00:44:15)

Continuity mistake: After they first meet Captain Willard looks at Dennis Hopper and says, "Take who away?". The red wind spinner to left of the Captain is in tatters, but a few seconds later when Dennis Hopper walks back to the front of the boat it's back together again. (02:30:20)

Continuity mistake: When they first meet, Colonel Kurtz says to Captain Willard, "Have you ever considered any real freedoms?" and the Captain's shadow appears on the stone column to the right of the Colonel. The Captain is kneeling down and hasn't moved from his spot, but when we saw this column before his shadow was not anywhere near it. (02:39:25)

Continuity mistake: When the Medevac helicopter first lands in the Vin Din Dop courtyard there are only soldiers on the left side of it putting the injured man in. In the next shot five soldiers and a Vietnamese woman appear standing on the right of the helicopter who weren't there before. (00:44:05)

Continuity mistake: Dennis Hopper stands on the back of the boat and says to Captain Willard, "Yeah, well, they think you've come to uh - , to take him away". If you look to the left of the screen you'll see a piece of seaweed hanging off the roof of the boat. When Dennis Hopper walks back to the front of boat it's gone. (02:30:15)

Continuity mistake: As Colonel Kurtz opens up the steel doors to the room where Captain Willard is being held we see thick black smoke engulfing the entire area behind the colonel. They cut away to the captain for few seconds then back to the colonel as he stands in the doorway, this time with no smoke at all behind him. (02:49:05)

Other mistake: Captain Willard hears the drumming as they approach the USO show at Hau Phat. He's sweating profusely but they don't put any sweat on Chief Phillips, Lance or Clean who, in the same heat, are bone dry. (01:03:15)

Audio problem: When they first meet Captain Willard asks Dennis Hopper who he is and he answers with a question, "Who are you?". If you look at the Captain's lips he answers the question but it was edited out. (02:29:55)
