Continuity mistake: When Bennett shoots Angel, the blood covers Kim's right shoulder. We then get a shot of Bennett, and when the camera cuts back to Kim, some of the blood has already dried up, in less then a second. (00:56:40)

Continuity mistake: Just when Alice jumps off the roof, pay close attention to the zombie directly to her right. You can see how he slows down and stops on the ledge. But in the shot directly afterwards, all of the zombies are jumping after Alice, and none are stopping where the roof ends. (01:00:10)

Continuity mistake: After the Tower Swing, Alice fires a bunch of quarters through two zombies, most of which end up in the wall around Luther. But when he has closed the door, we get an overhead shot of the wall, which has a bullet hole in it - and no quarters. (01:04:15)

Continuity mistake: In the scene after Alice bungee's off the roof, as she runs through the crowd of zombies towards Luthor, she fires the shotgun loaded with quarters once. As she gets closer he jumps to pull the door down and as all the zombies hit the door, you can see that it's solid, no holes. But when the shot changes to the inside view, the door now has quite a few holes in it. Not only were they not there before, but the door was open, so the quarters wouldn't have hit it anyway. Continuity mistake: During the fight scene with the Axeman, Claire slides between his legs while gunning him down with Alice's shotgun. But watch his axe. In the far shot he is holding it up, like he is ready to strike, but in the shot where he is falling, the axe is leaning on the ground all of a sudden. (01:04:15)

Continuity mistake: After the Tower Swing, Alice lands where she started out: in front of the garage with the UPV. Note the sign with the gun on it. But the door right next to it is closed. Behind that door is the UPV. Since everybody was in somewhat of a rush, it doesn't make sense that anyone should have closed it.

Continuity mistake: Alice is in the shower and draws her gun from the holster on the pole. She takes a couple of steps forwards, camera cuts, and she's suddenly much further forwards.

Continuity mistake: When Crystal is emerging from the water, she is almost out of the water completely in the long shot. As the camera changes to a close up, she's much further back into the water. (00:54:40)

Continuity mistake: When inspecting the plane for damage, the propeller changes position without being touched. The side by Alice goes from the 8 o'clock position to the 10 o'clock position. (00:34:30)

Continuity mistake: In Extinction, we see Kmart be handed the Alaska book and say "You're not coming?" to which Alice replies "Take care of the others." In the flashback seen here however, Alice replies "Take care of the others, Kmart".

Continuity mistake: In the after titles sequence, when Jill is addressing the group in the plane, she is walking with her hands on her back. When she turns around, heading towards the tail, in the long shot her hands are on her hips.

Continuity mistake: When the elevator hits the water, it starts to tilt over to one side. This contradicts the shot which shows it falling, where it fits very snugly in its shaft. (00:59:30)

Continuity mistake: When Alice is shining her flashlight into the tunnel, note that the shower column is on the right side of the tunnel entrance. In the shots on the outside of the tunnel, that same column is on the left side of the entrance. (01:04:30)

Suggested correction: From the shot in the tunnel, the shower column is on the screen right. When we see the group stood around the hole, the column is still on the right side.

Continuity mistake: Alice stabs Wesker in the head with the handle of the scalpel tilting over to Wesker's left side. In every shot afterwards, it's facing to Wesker's right.

Continuity mistake: Right after Bennett takes off in the plane, Claire throws a gun to Alice. Between the slow-motion shots, the gun switches position in mid-air before Alice catches it. In the shot of Claire throwing it, the barrel is facing to the right, meaning that it should be facing to the left in the shot of Alice catching it. But it still faces the right when she does. (00:58:30)
