Revealing mistake: When Eric Matthews is smashing his foot, he hits it the first time and it shows a close up. If you watch the small amount of blood when he hits it again they have used the same shot. (00:04:00)
Continuity mistake: Just before Amanda shoots Lynn her gun switches from her right hand to her left.
Continuity mistake: The drill that Lynn uses on Jigsaw's skull has a white piece of tape on the end of the drill bit a short distance from the end, presumably to mark the thickness of his skull so she knows where to stop. In one shot (during the drilling scene) the piece of tape moves a good two inches farther up on the drill bit, but it's back down in the very next shot. (01:04:05)
Continuity mistake: The tear in Jeff's face (after he gets stuck to the refrigeration machine) continually changes throughout the movie. It goes from oozing and bloody, to clean and almost normal looking several different times. There's no blood on his bathrobe, so he didn't wipe it off that way. That's a big enough tear to bleed a decent amount, so we'd see the blood if he wiped it off. (00:52:35)
Continuity mistake: After the scene with the pig gut pit, Jeff and the judge leave the room and the judge's clothes are suddenly not wet and almost clean. (01:17:15)
Continuity mistake: Right before Lynn says "I don't know what you know, but my marriage has survived more suffering that you could possibly imagine" her hair is all pulled back behind her ear. As she's saying that line you can now see that a noticeable amount is hanging on the side of her face and across her ear. (01:25:50)
Continuity mistake: The placement of the flashlight (or foot) moves between shots of Detective Matthews contemplating sawing through his leg. In one shot the flashlight is illuminating the ball of his foot, in the next it's pointed past his foot, then in the next shot it's shining right on his heel. Matthews isn't moving either, he sitting right there with the saw on his leg, crying and contemplating all the pain that he's about to inflict on himself. (00:02:30)
Continuity mistake: Lynn is in the corner of the room when Amanda is threatening her with the gun. The shot moves to behind Amanda when she raises her hand to her head and now all of a sudden Lynn is a good 4 or 5 feet to her right. (instant cut, not enough time to move).
Continuity mistake: The glowing "X" on the bathroom wall in the first Saw movie was very symmetrical, and the whole thing was filled in very well. During the Saw III bathroom flashback scene, when Jigsaw paints the "X" it's completely different that the one that shows up in Saw. It's not symmetrical at all, and the paint doesn't go on very thick at all. You can easily still see the wall behind the paint, but in Saw, the lines of the "X" are very full with no gaps.
Continuity mistake: Jeff sees the 3rd tape player (in the room where he finds the judge) and there's a shot of the player being held up by string. The player is hanging very level and the string is perfectly tied around the middle of it. Jeff climbs the stairs to get the player and now you can see that the player is hanging crooked and the string (where it's tied) is slanted towards the top of the player. (He didn't shake it lose or anything when he climbed the steps, the player is hanging from the ceiling).
Plot hole: When John is painting the doll's face he looks down at a photo printout of Amanda, you will notice the bruising/scratches on the side of her face in the picture. Please take note of her hair, shirt and overall demeanor. Now think back to the first Saw when Danny Glover had Amanda in the interview room. The picture that John Kramer is looking at is a picture taken of her in "that exact" interview, this is strange however considering that the interview took place after she escaped the jaw splinter, so how does John have a picture of her from the future? (00:51:40)
Continuity mistake: The blood spot on Lynn's left sleeve (from when Jigsaw has a seizure) constantly changes size and shape throughout the whole movie. (00:51:40)
Continuity mistake: When Lynn is operating on John's head, she the skin from the skull and then clips off some veins. Right when she asks Amanda to fill the syringe with alcohol to clean John's head, Lynn turns back to wipe up some of the blood, but there are no clips in place. A shot after that, we see her taking the syringe from Amanda's head and squirting the alcohol on John's head and the clips are there. (01:03:05)
Continuity mistake: When Jeff leans down to Lynn you can see she she has some blood on her chin and neck. The shot goes to Jigsaw, then back to Lynn and Jeff. Now her neck and chin have no blood. Shot moves to Jigsaw again, then back to Jeff and Lynn. Now she has blood on her neck and chin *and* blood from the corner of her mouth.
Continuity mistake: Look at Jigsaw's oxygen mask first time Lynn tells him "Look at my flashlight". The mask is next to his neck, lying flat on his shoulder. The shot cuts to Lynn for a second, then back to Jigsaw, now the mask is standing vertical and almost touching his cheek. (00:36:25)
Continuity mistake: When Lynn says "Please, please let me go" there are tears in both eyes and tear marks on her cheeks. 2 shots later (maybe 7 seconds) you see a close-up of her face and there are no tears or tear marks. (Jigsaw was holding her hand, so she couldn't have wiped them away, plus you can see Lynn the whole time (in the background) and she never wipes them off). (01:26:10)
Continuity mistake: When Lynn places down the drill there are 4 pieces of a broken mirror on the table in front of her. The 2 middle ones are standing straight up, but the outer ones are at various angles of slant. The next shot (a few seconds later) all 4 pieces are standing straight up and arranged very nicely. (00:49:40)
Factual error: Troy has several chains attached to his body, of which two are attached to his Achilles tendons. This is the structure of the leg that allows you to stand and move, so as soon as he ripped them out, he would have fallen over and been unable to walk.
Revealing mistake: During the "Rack" trap, look closely whenever there's a close-up of any of Timothy's hands or feet. It takes careful observation, but you can tell that many close-up shots have been horizontally flipped during editing. This was because the production only spent money on one prosthetic/prop hand and one prosthetic/prop foot for the twisting/bone-breaking effects, and in order to make it look like all four of his limbs were breaking, shots would be horizontally flipped. Thus the same prop/prosthetic could serve as both the left and right hand or foot. What gives it away is the blood pattern on the foot, which is perfectly mirrored between his two feet.
Continuity mistake: After Lynn gets shot her position against the wall changes between the far away and close-up shots. (look at the poster she's leaning against for a reference point, in the far away shot her head is between the 2 legs on the left, but in the close-up her head is on the very left leg and you can easily see the right leg.).
Answer: Adam didn't just lose because he let the key get drowned. He had the two hacksaws and he broke his own in a hurry. And there was also the toilet lid that he could have used to smash his foot like Eric did in Saw III. Jigsaw never said that improvising or thinking outside the box was against the rules. So even if you wipe the key out of the equation, Adam still has at least two other ways to release himself from the chains.
