Visible crew/equipment: Watch the scene where Frank gets killed. In the last shot of this scene, we see a soldier standing over the dead body. Keep an eye on the right side of the screen as the camera slowly moves away and you should see one of the cameras and a cameraman on the ground next to a large metal object.
Visible crew/equipment: At the beginning, when Michael kills the first security guard in the basement, just before he dispatches the second one, you can see a crew member's arm reflected in a window hitting the light shade to keep it swinging.

Visible crew/equipment: When Picard, Data and Worf are on the planet, just a few shots after they leave the shuttle, Picard drives a sharp left curve. If you look at him in this shot, it is not Patrick Stewart sitting on the driver's seat, but his stuntman, who is not even wearing goggles. (00:12:45)

Visible crew/equipment: When we first see the "Tooth Fairy" he is lifting weights in a room full of mirrors. If you look close, you can see one or more crew members reflected in some of the mirrors. The most obvious is a man standing, whom I am guessing to be the director or a grip.

Visible crew/equipment: After Chris Rock and Anthony Hopkins hit the 'panic button' and escape from the 'triple cross' in the church, they cross an old wooden bridge. As they cross, the loose boards supposedly knock off their GPS device from the underside of the car. As they approach the bridge you can VERY clearly see the GPS device waiting on the bridge ahead of their crossing, only to be bounced around under their car as they cross & then zoomed in on by the camera.
Visible crew/equipment: When JD shoots the zombie female in her leg you can see the square outline of the pack behind her leg that 'explodes' and makes the hole in her pants. (00:38:10)
Visible crew/equipment: The narrator explains that the traffic of drugs is a business just like any other. As we hear the words 'the police receive their part', a boom mike is visible at the top of the screen. (00:48:40)
Visible crew/equipment: In the ice skating scene at the end of the movie, Shelton and others are saying good bye to Smoochy. Buggy fires, and the camera goes to Shelton falling to the ground. If you look at Shelton's head, you can see the special effects tub that shoots air (or something) out, to give the impression a bullet has just gone into Shelton's suit.
