Visible crew/equipment: Towards the beginning, when the Taliban steps on the landmine, you see the DEA agent getting hit with dirt and blood. In the center of the shot, you can see a few drops of blood stick to the camera lens. Incredibly easy to notice, so I'm not sure how this one slipped through.
Visible crew/equipment: You can briefly see part of a camera and camera man at the right of the screen after Tobias and the counselor finish their "scene" during therapy.
Into That Good Night: Part 2 - S9-E24
Visible crew/equipment: When Bev hears "movement" on the baby monitor everyone rushes toward Roseanne on the stairs, and just as Roseanne comments that they're all "scary enough individually," we can see the second camera appear at the left side of the screen. Then, when Bev walks through the bedroom's doorway, there's movement of the boom mic at the top left corner of the screen, and when DJ turns on the camcorder the end of the set is visible from the POV of the camcorder.
Visible crew/equipment: Joe and 4 fellow stagecoach passengers are in a cave. A fist-fight between Joe and another man ensues just outside the cave. At one point, a rubber tire is visible in the lower left of the screen. (00:40:15)

Visible crew/equipment: When The Patrol is on its way, one of the boats tilts over and some soldiers are screaming they can't swim. At some point during that scene there is a wide shot with in the background the other side and a skylight. Right in the middle of that shot on the other side you can see a man walking away wearing a orange vest or jacket of some sorts. Might be the pyro-guy. (00:33:15)
Visible crew/equipment: The Great Santini makes a vase of flowers appear on a table. Before he holds up a cloth to conceal the vase as it appears, the tips of the flowers can be seen beyond the far edge of the table. As he pulls the cloth away, there is a quick glimpse of the hand of the crew member who put the flowers on the table. (00:11:00)

Visible crew/equipment: At the end, as KITT goes to jump the camper, there is a set of what appears to be mirrors to hide the obvious ramp.
Visible crew/equipment: While Ed, Chico, and Louie are at the diner, when Anita pours Louie some coffee Chico comments that she's a strange girl, and in the next shot the boom mic dips into view at the top of the screen. (00:14:30)

Visible crew/equipment: When Lee is being interrogated, at one point (after the detective says "Are you crazy?") the detective walks towards the large two-way mirror and you can see a clear reflection of a cameraman in the lower part of the mirror. (00:27:31)

Visible crew/equipment: When Flores discovers her cell phone is missing from the hole in the wall she goes nuts, slams the stall door and kicks it in. After she is taken away there's a closeup of the stall and we can see the metal spring that was duct-taped to the bottom of the wall inside.
Visible crew/equipment: Near the end of the episode when Harry says "there will be a 10 minute recess" a boom mic is visible.
Visible crew/equipment: In the rec hall, when Kensi feigns sickness while posing as a cadet the gunman jumps off the stage, and as Kensi bends over the small rectangular bulge of her mic pack can be seen at her back. (00:30:25)
