Visible crew/equipment: After Bud wakes up all alone in Bligh's cabin, he soon realises all the doors are locked. Then, just as Bud rushes over to the stern window (particularly the second time), a seated crew member is visible at the bottom right corner of the screen; we can see the movement of a leg and two hands (so, it's not a mannequin).
Fine Feathered Finks (1) - S1-E3
Continuity mistake: When Batman starts the Bat-Turn, the parachutes are shown deploying on a 6-lane major roadway (3 lanes in each direction) in a very open area, but after completing the 180-degree turn, the car is shown driving away from the crumpled chutes on a much narrower side street among tall buildings. Continuity mistake: As Rhodey and Tony are walking by a building, Rhodey tells Tony to not worry, as in two years he can fire Stane. The first view shows their shadows directly under them, like the sun is at its zenith. Visible crew/equipment: When the truck carrying the caged gorilla pulls into the alleyway, just as Hans and Carl exit the truck and walk away, the blue-sleeved arm of the cameraman/crew member is visible as it moves upward, at the right side of the screen.
Rising (1) - S1-E1
Continuity mistake: When they first arrive on Teyla's planet, the DHD is close to the gate. When the wraith attack, the DHD is much further from the gate.
Continuity mistake: When Tommy is escaping into the mountains to try and help, a small bit of his jacket tears off onto a tree branch. MacGyver later sees it, but the branch that the torn piece is on looks entirely different. (00:21:10 - 00:22:10)
Revealing mistake: When Syd's Dad rescues her in the car park, he starts shooting at the other car. After firing one shot the slide of his gun locks back (indicating it's out of bullets), but when the angle changes he carries on firing with no problems. (00:43:20)

Continuity mistake: When Wilson jams the pistol in Oliver's face, Wilson is holding the gun in his right hand. When the shot changes, it flips - he is suddenly holding it in his left hand, and his previous un-scarred left eye now has some scrapes around it.
Visible crew/equipment: When Zaphod says his first line "I certainly didn't survive.", you can see the person who plays his third arm standing behind him.
Continuity mistake: The package Nigel holds in his arms near the beginning of the episode completely changes between shots.
Dear Diary - S4-E20
Continuity mistake: While it's forgiven that the Generals usually have subtle changes through episodes and the series, in this episode we see a General with extremely inconsistent graphics when Bo and Luke deliver the counterfeit money to Winston. The 0 is off and the bottom of the 1 is extremely long.
Continuity mistake: When Starfire asks what is the Maximum 7, her, Robin, Raven, and Beast Boy are standing together looking at Cyborg in a wide shot. Raven is slightly behind Beast Boy but to his right, and her body is facing to her left. But then it cuts to a close shot from in front of the Titans and suddenly Raven is on Beast Boy's left and right beside him, with her body facing her right. (00:03:50)
Other mistake: After the teens leave the command center, Rita sees this and orders the creation of her Putties. Her henchman slaps down a few things of clay into a mold loosely, that then show the perfect figurines of the putties coming out of them. However with the shape of the mold, there is no way that the clay would come out in shapes like that or with that detail. (00:11:35)
Continuity mistake: Phoebe is going into the school in one scene, but as the camera moves she vanishes while on screen.
Visible crew/equipment: After Paris hails Tuvok about the problem in the mess hall, a brawl breaks out. When the new Hirogen Alpha enters the galley to access the com relay, just as this Alpha removes the front panel in order to transmit a signal, the actor's green tape mark is visible on the floor. Suggested correction: Given that these are literal monsters and a witch living on the moon after being imprisoned for 10,000 years, I don't think we can say for certain what their machinery will do.

This is speculating too much. Practically making the same argument that it's OK because magic. The general physics of what is being displayed on the screen doesn't match up with how the clay fits into the mold even in the slightest.

What Quantom is saying is that they are using a press die to create the mold for the putties and no matter how you look at it, the volume of clay going into the mold is not enough to create the detail seen on the putties.

It's not just that, but the shapes don't match up at all. He does the equivalent of putting clay into a round hole and getting a square out of it.
