Visible crew/equipment: When we see Marsha hanging outside she is supposed to be hanging from a wire connected at the roof above her. However we can see ropes on each side of the screen which she is tied to. (01:04:40)
Visible crew/equipment: When Mika brings Katie upstairs after investigating another noise downstairs, you hear a thump come from downstairs. Mika turns the camera down the stairs, and you can see a crew member scrambling to get out of the shot.
Visible crew/equipment: Sometimes, especially at night, you can see the lights of the equipment reflect off of Tony's glasses. We know this isn't the moon or a car light because for one thing it's the wrong shape and when Rudolf is sucking blood out of the cow there is no moon or car nearby. So there is only one place the light could have come from, and that would be the crew equipment.

In one shot, Graham steps more carefully and glances down; the camera is in front of him moving as he's moving. And for a couple of seconds, the camera dolly tracks are visible behind him, just to his left.

In the upper right hand corner above the saturation chamber you can see the microphone fully in frame and "hovering" as they re-enter that room.
