Revealing mistake: When the blind guy puts the Seal or key into his chest you can see for the first two times he "hits" it to his chest, that he is not hitting himself at all.

Continuity mistake: In the first PA, Katie says that the "ghost" was in front of her bed, however we never see it in PA3.

Continuity mistake: When Adam and Gordon are talking in the beginning, Adam's hair moves between shots. (00:03:40)

Bug: Isaac seems to be immune to a great deal while swinging his fist. One of those things is suffocating. In Zero-G Therapy, if one carries out melee moves constantly, when Isaac runs out of air, he will not expire until he stops moving.

Bug: This is a very easy glitch in Chapter 1. Simply start a Cleared Game save game and go along until you enter the room where the first Slasher drops to the ceiling triggering the start of all the mayhem. Because you started a Cleared Game save, you start with your previous guns. After walking into the little hallway where the console is, shoot the vent on the far end of the hallway. A Slasher will come out. Run out of the hallway and into the room with Kendra and the rest. The Slasher will crawl through the vents and drop into the room. It stalks Isaac, but ignores everyone else in the room. It can be killed like a normal Slasher.

Bug: Rarely, Isaac can walk through a charging Brute, by charging the Brute itself and running between his arm and torso. Regardless, this does leave Isaac with some health loss as he can get tossed back like he was hit anyway.

Bug: When in the Zero Gravity area on Chapter 7 (Into the Void) where you must destroy 4 free floating asteroids (also known as anomalies), there is a chance that the asteroids might not all appear. This in turn means that gravity cannot be re-enabled in the room and therefore also means that you cannot progress through the level. In this case, you must reload your previous checkpoint to continue.

Goin' Down the Road: Part 2 - S4-E22

Visible crew/equipment: The reflection of the cameraman can be seen in the vanity mirror as Spike and Caitlin hug, and again in the glass door, during Kevin Smith's hurried flight from Caitlin.

Continuity mistake: When Heather is in the elevator, and the monster slashes it up, the hole on top changes to a bigger cut in the next shots.

Revealing mistake: When Heather is in Silent hill, (right after the Mannequin part.) and talks to Alessa's mother, if you look at her nostrils you can see two rings of clean skin. (the rings are there for make-up reasons) but then after the flashback they are gone.

Revealing mistake: When Rose is in the mirror, her hand is under the husbands, when she removes it his hand moves along with it, but his real hand doesn't move.

Visible crew/equipment: When the cart falls over with the nurses, when it hits the ground, the camera shakes.

Continuity mistake: When Heather comes home to see that her dad is missing. She sees the Come to Silent Hill, and the lamp is some what on the chair's arm. When the cops come in the room the lamp changes.

Continuity mistake: When Pyramid Head cuts the prisoners arms off, one falls in front of Heather, the distance between heather and the arm change between shots.

Audio problem: When Heather is reading her dads note in the car to Silent Hill, the letter and the voice over do not match. (the second paragraph starts off by saying: "But you desire" and the last word right before "I love you" is "place" but through out the reading you do not hear those words.

Other mistake: While Dennis is editing one of the video tapes, there's one on a table labeled "89" even though the film takes place in 1988.

Revealing mistake: While driving the Mercedes to the house of grandma, the gauges on the dashboard show speed is zero and the engine is cold.

Revealing mistake: On night 21 the time-stamp changes from 11:59:56 PM to 11:59:57 AM despite it not having turned midnight yet.

Visible crew/equipment: When Mika brings Katie upstairs after investigating another noise downstairs, you hear a thump come from downstairs. Mika turns the camera down the stairs, and you can see a crew member scrambling to get out of the shot.
