Bug: In the beginning training missions (the white rooms). In the mission where you are trying to find a door and guards are walking around underneath you take out the guard in one of the buildings and stick him in the left hand corner (facing away from the door) then throw 1 or 2 grenades at him (this is the only time in the game this will happen). When they land on him and explode all the walls will be covered in red paint (presumably blood) even on the ceiling - this should not be happening.
Bug: In level one, after you receive the elevator code and use it to open the elevator, look at the reflection of the elevator doors on the back wall. Even though the elevator doors are open, the reflection shows the elevator doors are still closed.
Bug: Applies to the SNES. Using the character Raiden, upon finishing the last level of the double team fight. Upon doing his fatality, Gorro will drop down in a glitched color form.
Other mistake: During one of the games numerous 'radio-interviews', a local sheriff is calling to report, that he's been following an unidentified object for a while and that he had to 'hit the breaks' of his car to avoid collision. The subtitles should of course have read 'hit the brakes' instead of 'breaks'.
Continuity mistake: In the last cut scene for the battle on the bridge, there is a shot where Indy, Willie, Short Round and Mola Ram are hanging on for dear life. There is another scene and after that Willie and Short Round are gone from the shot when they should be in it.
Continuity mistake: When leaving Parm in the beginning of the game to travel to Java in Leck mines the number of carts on the train in the stations is three but when going through the city it's one and then three again at the Leck mines.
Audio problem: In the scene where Tidus and Yuna are conversing at the Farplane, Yuna says something slightly offensive about Tidus' mother, to which he replies, "Whoa there, that's my mother you're talking about!" Almost immediately after, you hear Yuna say, "oh, I'm sorry," but her mouth doesn't move.
Factual error: In the scene where Snake is ambushed by the M1 tank we see a view from inside the barrel and that it is rifled; the Abrams has a smoothbore cannon, not rifled.
Continuity mistake: Before you begin level 2 a cutscene shows the SFOD-D troops boarding the helicopter with you, they are all carrying G2A2 assault rifles. However when you actually start level 2 the SFOD-D troops now all carry RPL SMGs instead.
Continuity mistake: During the second case, Phoenix says that Maya was involved in a murder when a spirit she was channelling took control of her body and shot someone dead. Phoenix and Maya both seem to have forgotten that he discovered that another woman had knocked Maya out during the channelling and committed the murder herself. In fact, he had to prove this in court to prevent Maya from being executed.
Continuity mistake: Near the beginning of Final Fantasy II, Firion, Maria and Guy witness Scott dying in the basement of the pub in Fynn. However, at the end of Soul of Rebirth, the Emperor says that Scott died while defending Kashuan Keep.
Continuity mistake: During cutscenes, Templar is holding his M-82 Assault Rifle, no matter what weapon is equipped at the time.
Bug: At the beginning of Blobolonia, the blob's planet, run all the way to the left until you can't go any farther. Toss a jellybean at the left side of the screen. If you did it right, the screen will flash the final boss screen for a brief moment. Now there will be no enemies at all on Blobolonia.
Audio problem: When you're playing Blaze, at the end of almost all the Boss Stages, you hear Eggman say 'You'll regret this' but the subtitle at the bottom of the screen says 'I won't forget this'.
Continuity mistake: When inside Mephisto's realm, Jean Grey and Nightcrawler are both kept in cages and Blackheart informs the player that only one of the heroes can be saved, but the other hero will die. If the player chooses to save Jean Grey, then Nightcrawler will fall into the pit, and for a short while aid Mephisto before sacrificing himself. After the game is over, one of the cut scenes will say that Mystique, upset over the death of Nightcrawler, kills Professor Xavier. When it shows Professor Xavier's tombstone, Nightcrawler is seen standing next to it.
Bug: In Episode II, as Obi-Wan lands on Geonosis, in the shot when the ship comes down, its shadow does not grow as it should.
Plot hole: When Sonic and Tails land on the Egg Carrier, Tails reminds Sonic that they have to go look for Amy. But Sonic never told Tails that Amy was on the Egg Carrier, and Tails was not there to witness her kidnapping, so how could he have known?
