Continuity mistake: While Joan and Sherlock are chatting, Joan is trying to make peanut butter and jelly sandwiches, but the shots alternate between the two and Joan repeats her movements, wipes the peanut butter over and over on the bread, and changes her mind with the jelly bread.
Revealing mistake: It's a valiant effort, but the scenes of Sherlock & Irene in a London street are shot in New York. Not only is the architecture wrong but you can see a U.S. pedestrian crossing signal over Irene's shoulder. (00:16:15)
Factual error: The number plate on the black cab is GW22 ARY. This won't happen until the year 2022. (00:28:20)
Continuity mistake: When Sherlock first accuses Irene of working for Moriarty, he pins her to a bookcase. When she shoves him away and breaks free, her hair ends up arranged with some strands in front of her. During the following minute, those strands repeatedly switch back and forth from behind her shoulders to in front of them. (00:38:25)
Continuity mistake: When Detective Bell receives a phone call from Gregson in Holmes' house, he rushes upstairs and bangs on the bathroom door - Sherlock is inside, he said he wanted a shower. Bell rattles the handle, but finds the door locked, so he kicks the door in. But Sherlock didn't lock the door. The lock is above the handle and Sherlock did not use it.
Continuity mistake: When Holmes finds Lestrade in the bar, Lestrade is seen putting his glass down, but in the next shot he's still holding the glass up. The items on the bar change around too, depending on the camera angle.
Character mistake: Clyde is a tortoise, but Joan, after finding Sherlock, put him in her bed, brings him down and calls him his "pet turtle." (00:14:08)
Visible crew/equipment: Sherlock and Joan are sitting on a street restaurant. The restaurant glass door opens in the background and you can see a light box reflected on it.

Continuity mistake: At the start, when The Woman first fires at the door the holes are quite dark and splintered. Then when she moves to pick up the phone the holes look far neater, like they've been drilled.
On the Line - S2-E9
Continuity mistake: When Holmes and Watson are arguing in the kitchen, Watson puts her mug and phone on the counter. A few times during the scene, the mug switches which side of the phone it's on. The first time it's plausible Watson moves it when off camera, but subsequent to that, we can see enough of Watson to tell she doesn't make enough movements to change the position of the mug.
Continuity mistake: When Cloe and Watson are talking and drinking tea in the kitchen, the positions of the cups and the teabag string change depending on the camera angle. Not just where on the placemat, but the orientation.
Continuity mistake: Watson came to Mycroft's place, spent the night and was awoken by Sherlock (this episode) the next day. Somehow she had a nightie and a brand new outfit. (00:00:05)

Factual error: The 1-ton electromagnet is supposed to have moved only the bullets without affecting the steel elevator car at all. This is impossible.

Character mistake: Sherlock says "Cherchez la femme" but pronounces "femme" as "fem" rather than "fam." In earlier episodes, he has been seen speaking fluent French, and so would be unlikely to make so basic an error. (00:16:30)

Factual error: In the scene in the hospital, the guy who drove his motorbike into a van has an IV canula in his right hand, but the IV pole and the IV bag are on the left side of the bed, which means the IV line has to be long enough to go up to the bedhead, then behind the bedhead and then up to the bag, leaving no way to check for air bubbles.

Continuity mistake: When Bell and Holmes are looking at the files, there's an open book next to Bell. When he starts texting, with both hands, the book becomes closed in one shot and then back open in the next.
Factual error: Holmes shows an epinephrine auto-injector, but it is only a training device without any needle or medicine: 1. The real epinephrine auto-injector has a transparent housing to enable the user the check of the solution inside the cartridge. The shown "device" has only a grey, opaque housing, as the training device has. 2. The real epinephrine auto-injector has an orange-colored needle guard in the form of a hollow body. The shown "device" has only a finned dummy. (The real epinephrine auto-injector (single use device) is sold with a training device (reusable) to let the user practice the application once or multiple times). (00:25:50)
Continuity mistake: At end of the episode, when Sherlock leaves Detective Bell outside the 11th Precinct, Bell had a book and an envelope of which Bell puts the envelope in his pocket leaving the book, the next shot shows Bell turn and throw the envelope in the trash and the book disappears. Character mistake: Sherlock and Joan are looking at video footage of a restaurant called "La Porte Blanc", which is incorrect French. The right way is "La Porte Blanche."
Character mistake: The American Flag is hung improperly on the wall. The Flag's union (stars) should be at the top, to the observer's left, but it's to the observer's right. (00:06:38)
