Continuity mistake: While talking Paige picks up a small white Chinese food takeout container to close it and puts it back down on the desk. She picks up a the larger container that has the red pictures and the word enjoy on it and starts to close it. In the closeup she has the small white container back in her hand.
To Catch a Predator Predator - S5-E5
Continuity mistake: When Joan is sitting on the couch folding ties while Shinwell tries on a shirt, she has a folded up blue tie with white dots. When the camera cuts back to her and she's now folding a gray tie, you can see the blue tie with white dots, unfolded, lying on the clothes. Previously there was no tie there. And in the next shot of her, she once again folding the blue tie and the gray tie can't be seen.
Continuity mistake: When Shinwell and Sherlock are playing chess, the captured pieces move around in various shots. For example, Shinwell takes Sherlock's knight, he places it closer to him than the pawn. In the next shot the pawn is closer to him. It changes a few more times as do the captured white pieces.
How the Sausage Is Made - S5-E8
Continuity mistake: When Sherlock puts down the meat sample in the cup, you see the toothpick tilt over to the side. In the next shot, the toothpick is straight up and down. In some shots, the meat sample in the cup are gone.

Continuity mistake: The note Watson picks up changes when it switches to a close up view of the writing. For example, you can see one line ends with "you when" and another line ends with "not" (being underlined). In the close up the corresponding lines end with "you when I" and "not to" (not still being underlined).
It Serves You Right to Suffer - S5-E9
Continuity mistake: When Joan accuses Agent Whitlock of gathering intel to Rip Off/ambush/rob the gangs, a woman enters the hall behind her. Moments later, The Woman disappears having never walked past Joan or Agent Whitlock. She didn't open any of the doors in the hallway either since she was walking straight down the hallway.
Continuity mistake: When Marcus and Joan are watching Gephardt's video, he says he's taken lives. At this point the play bar is by his fingers. It cuts to Marcus and Joan, and we hear the video continue to play for more than 12 seconds. When it cuts back to the video, the play bar is nearly in the same position, when it should be well past his hands and near his elbow.
The Ballad of Lady Frances - S5-E17
Continuity mistake: While talking with the guitar expert, Joan places the guitar down on the sofa with the fretboard facing away from her. When the guitar expert comes and sits down next to her, the guitar's fretboard is suddenly facing her.
Character mistake: When they've worked out that the kidnappers were changing flood maps, Harlan says he "doesn't get why anyone would mess with flood maps in the first place." Looking at the maps it's blatantly obvious that the reworked flood zone excludes a chunk of land, and given property prices in New York it doesn't take a genius to figure it's at least connected to real estate, insurance, etc. Harlan is demonstrably a genius, and could certainly have figured that out for himself.
Red Light, Green Light - S7-E4
Other mistake: The crash at the start is staged - the semi truck is driving at 20/30mph, is T-boned by the van, and stops instantly. No screech of brakes, no allowance for the driver's reaction time. In the shot of the impact it's moving at barely walking pace, hence being able to stop so quickly.
On the Scent - S7-E9
Continuity mistake: Sherlock and Marcus visit a suspect's house. There are a bunch of cell phones on a console. One is gold with the name "'Mandy" on it. In a wide angle shot the gold phone is by itself next to some other black phones. Next closeup shot you see the gold phone is on a different position behind some other phones, and a green phone is added to the bunch. (00:18:40)
Unfriended - S7-E11
Character mistake: Odin Reichenbach talks to Sherlock about "doing the irrevocable", but pronounces it "irrevOWEcable." It should be pronounced "irrevacable" in both British and American English. He's supposed to be a highly intelligent, highly educated person - not a mistake he should make.
Suggested correction: Not necessarily. A lot of people have strange vocal tics where they pronounce a certain word in a certain way, usually without even realizing it. My grandfather was a smart guy, and he always mis-pronounced "toilet" as "ter-let."
Character mistake: Sherlock tells Odin at the bridge where he framed him for his own murder that they are opposite poles of a magnet - they repel. That's wrong - opposite poles attract each other.
