Continuity mistake: When Regina talks to Rumplestiltskin, he is holding onto the bars in his cell. In the beginning of the scene, his left hand keeps clutching different bars between shots. However, according to 1-20 "The Stranger", Pinocchio's story was not added to the storybook until later after August came to Storybrooke, several months later. This is confirmed in 4-13 "Unforgiven", where Regina and Henry look at the exact same picture, and notice that the paper is different than the rest of the book, because this particular story was added by August. Continuity mistake: When Peter Pan removes the enchanted black leather cuff from his arm, he uses his magic to transfer it to Mr. Gold's wrist. However, as he sends Mr. Gold flying across the room, there is a close-up of Pan's arm, where the cuff is back on his arm, next to the normal brown cuff that he usually wears. (00:21:20)

Plot hole: When talking to Hook about the photos, Emma states he could have photoshopped them to make it look like she and Henry were in Storybrooke, however she had taken them directly from Henry's camera and had them developed. So Hook had no way to photoshop the pictures from inside the camera. (00:27:00)

Continuity mistake: When the Vault of the Dark One opens, there is a wide shot where Neal turns around, and you can see that the area around the circular vault is covered in snow. In the next shot, a large area around the vault is free from snow. The amount of snow around the vault keeps changing during the rest of the scene. (00:26:45)
It's Not Easy Being Green - S3-E16
Continuity mistake: When Hook is at the beach with Henry, after he says to him, "Fair enough, mate", the pendant on Hook's necklace goes from resting against his chest, to being partially tucked under his waistcoat. In the close-ups, the pendant is completely covered by the waistcoat. (00:26:50)
Revealing mistake: When David is letting Henry drive his truck, there is a close-up of Henry, followed by a wide-shot of the car where the tires are screeching, and you can see that the one who is actually driving is Jared Gilmore's stunt double. (00:30:15)
