Other mistake: First we see Jamie in a Ford CVPI with license plates 7554 like a couple of times before in the show, a minute after he's seen exiting a 2015 Ford Taurus with plates 82399. He answers a 10-85 while supposedly driving in the CVPI but somehow he manages to change in less than a minute to a Taurus. We see the exterior of the car, but the car's interior corresponding to the Taurus is seen a few seconds earlier. (00:03:35 - 00:04:20)

Continuity mistake: Jamie is arresting a subject after he drops a knife. Jamie kicks the knife away from the subject and it lands next to the brown box and under the red wording on the box. In the next frame, the knife is now just behind Jamie's right knee. (00:05:10)
Visible crew/equipment: When Eddie and Jamie stop to help the woman holding the boy, in the exterior shot as Jamie gets into the back of the RMP with the young boy, the reflection of camera equipment is visible on the window. (00:04:55)

Continuity mistake: The Chinese food containers on Eddie and Jamie's table change positions, and the quantity of containers also changes. (00:18:08)

Factual error: Police Officer Troy was taken into a room by Sgt Regan to be interviewed. He has one Sgt stripe on one sleeve. He's a police officer, not a Sgt.
Fog of War - S10-E14
Factual error: During the funeral towards The End of the episode, both Jamie and Eddie are wearing white shirts with their dress uniforms. In the NYPD, only Lieutenant and above wear white shirts, like the show has portrayed. The color doesn't change for the dress uniform. (00:38:35 - 00:41:14)
Continuity mistake: The Reagan men sit at the table for a drink. Jamie's glass moves towards/away from him in different shots. (00:23:15)
Continuity mistake: Toward The End of the episode when the older mobster is walking from his car down a narrow alleyway toward home, he passes under a low hanging outdoor light fixture. When he is accosted by the other mobster with a gun they are standing with the light fixture between them. There is no movement, but when the police arrive, the light fixture is well behind the guy with the gun.
Factual error: The badge the ATF Special Agent is displaying on his suit jacket is wrong. It's the wrong shape and style of an actual ATF Badge. (00:02:57)
Continuity mistake: Jenko and Witten confront a guy creating a commotion in a market. The felon runs past them out to the sidewalk where, somehow, the two cops are waiting and grab him.
Character mistake: Officer Rachel Witten tells Eddie the gun she has belonged to her dad when he was on The Job. In episode 15 of season 8 Gormley tells Frank that Witten's dad was a FDNY Battalion Chief and her uncles were police officers. In episode 12 of season 9 Officer Witten's dad Sgt. Scott Witten speaks to Frank about being on The Job. It's unlikely and not normal for someone to become a firefighter after being a police officer or vice versa or do both jobs.
Other mistake: Eddie and her partner respond to a store selling sandwiches with contradicting prices. For example, grilled cheese is listed for $3.99 and $6.25, and tuna melt is $5.50 and $5.99, and BLT is listed twice on the same sign. (00:03:10)
Continuity mistake: During the family meal, Jaime takes a bite of food, then immediately in the next shot, both arms are down, and he is talking and not chewing.
On the Arm - S12-E11
Character mistake: Lt. Gormley mentions a Captain (Captain Terrell) taking items On the Arm that is a 33-year veteran. Captain Terrell tells the PC that when she was 12 her brother was wounded in Iraq. If she has 33 years on The Job, there's no way she could have been 12 during the war in Iraq. The Vietnam War would have been more appropriate for the age and background of Captain Terrell. (00:00:21 - 00:14:33)
Continuity mistake: Jamie runs to a car which has crashed and has a light pole across its roof. The light pole disappears in the closeups as Jamie struggles to free the driver.
Character mistake: After the explosion, Danny calls for a medic. At the hospital Baez ask Danny if the reason he called for a medic instead of a bus was it because it reminded you of something that happened. Danny was a Marine, so if he was having a flashback he would have called for a corpsman instead of a medic. Medics are in the Army, not the Marine Corps. (00:06:50 - 00:54:22)
Character mistake: Eddie is confronted by Captain McNichols after being caught in her office. Eddie responds by calling her Sarge, which is an incorrect title remark referring to a Captain. (00:38:46)
Continuity mistake: While Danny and Waylon Gates are with Rodriguez at the safe house, showing him the photograph of his daughters, during this scene there is no picture hanging on the wall beside the door, but as they lead Rodriguez out in the last shot of the scene, a picture suddenly appears on that wall. (00:37:30 - 00:39:20)
Visible crew/equipment: At the safe house, while Danny and Waylon are alone with Rodriguez they "convince" him to call off his men, and in the last shot of this scene as they head out the door, the reflection of a crew member (presumably the camera operator) becomes visible on the picture hanging beside the door. (00:39:20)
Continuity mistake: When Danny and Baez confront Carmine to clear a few things up, Carmine tells them to call his lawyer. Just as Carmine clicks his key fob and his car explodes, Carmine's coat is entirely unbuttoned, but when he's blown backwards and slams onto the ground, in the first overhead shot of Carmine lying on the ground, his coat is buttoned up.
