Other mistake: Danny supposedly handcuffs an at-risk teenager. When the police officer takes her away to a cell, the girl isn't handcuffed. The handcuffs are just in her hands behind her back. (00:52:00)
Continuity mistake: During the stakeout of the apartment building, in the exterior shots of the Clyde Motorcycles shop, Danny is peeking out through a ripped paper opening within the letter "D" in Clyde. However, in the interior shots the black tape letters of the word Clyde differs. The paper is ripped between the two letters "D" and "E" and that space within the "D" is blocked with molding, making what is in the exterior shots quite impossible. (00:15:40)
Continuity mistake: During the takedown at the apartment Danny is entering the building wearing hoodie and the shield around his neck, going upstairs, the next shot he is in shirt tie and vest at the staging area, then he is back in the apartment in the doorway in his hoodie, then he is in the street in vest and tie. (00:20:48 - 00:22:21)
Suggested correction: Danny is not one of the 4 men who follow Maria Baez inside and up the stairs (at 00:20:40). They are John Knight and his 3 men, all plainclothes with their shields hanging from necks. Earlier, right before Baez and the 4 men head out to the apartment, John Knight is standing near his 3 men inside the Clyde motorcycle shop (at 00:18:46 for example). Rewatch please.
Visible crew/equipment: When Danny gets off the phone he tells Baez that a Port Authority cop has detained their guy, and at the start of the next shot facing Baez, the reflections of the moving boom pole/mic and the boom operator are visible on the glass, at the right side of the screen. Factual error: The NYPD Sergeant in uniform speaking to Danny only has Sergeant stripes on the left sleeve of his uniform. An NYPD Sergeant has stripes on both sleeves. (00:03:14)

Continuity mistake: When Erin is in the ladies' room walking toward the door, she finds Anthony "lurking" in the hall, and Erin's eyeglasses which are hanging from her crew-neck top go from facing her right side, to her left side, then back to her right side, between shots. (00:31:55)
Visible crew/equipment: After Frank tells Garrett to set up the meeting with Thomas and agrees to play nice, when it cuts to Erin as she walks toward Zach and the AG, the T-mark and white tape mark are visible on the floor in front of the two men. (00:01:50)
Visible crew/equipment: When Eddie comments that the Rough Tough Cream Puff made a collar, she and Jamie walk up the hallway, and before Eddie reaches the doorway we can see the red tape mark on the floor as we hear Marcus brag, "mad fast." (00:15:55)
Continuity mistake: Danny and Jamie are at the pistol range shooting targets. The vantage point from down range shows them both shooting tight shot groups. However, when they are finished shooting, they hold up their targets; their shot groups are not as tight as the down range perspective. (00:34:01)
Continuity mistake: As Danny and Jaime are at the range shooting near the middle of the episode, their targets from behind and the grouping of shots they take are really tight together. When they retrieve their targets a minute later, the grouping from the front is completely different and further apart than what was seen from behind the target.
Continuity mistake: At The End of the episode, a screen shot of Nicki occurs. A day or so earlier, she had been kidnapped and pistol whipped across the face. But this screen shot shows no bruises or swelling or anything, despite being hit so hard she fell over in a chair just a day or so before. (00:40:00)
Visible crew/equipment: After Danny and Baez get the hospital physician's rundown on Joe's condition, when the partners reintroduce themselves to Joe the reflection of the boom pole/mic is visible on the glass behind Danny.
Factual error: At The End of the episode, Mayor Poole is on TV discussing the recent police shooting. He refers to officer Meeks as a shooting death. Officer Meeks was wounded in the neck with spine injuries and retired medically.
Visible crew/equipment: When Erin walks into her office and finds Robert waiting for her, as she walks past him toward her desk, the green tape mark is visible on the floor near Robert's feet. (00:01:00)
Continuity mistake: As the couple with their two children are walking down the street, the daughter, Sophie, is holding latex helium balloons which are the transparent type, but after she and her mother are struck by the reckless drunk driver, when the balloons float away they are the opaque type. Continuity mistake: Franks makes a comment that there have only been 18 deaths last year in New York, but Danny on his own closed more than 30 in the season before.
Visible crew/equipment: When Danny tells the M.E. what the victim shouted prior to being hit by the car and what the word means, the reflection of the moving camera is visible on the glass. (00:07:25)
Visible crew/equipment: At the start, when Erin is interviewing Miranda they take a break, and just as Miranda says she's starving and stands up, the red T-mark is visible on the floor in front of Anthony's feet. (00:00:55)
Visible crew/equipment: After the family dinner, when it cuts to Jamie standing at the sink loading the dishwasher, the green T-mark is visible on the floor behind his feet. (00:29:10)

Continuity mistake: We can see Jamie driving a Ford CVPI with plates reading 7554, however when he appears in the following shot in the court, where he was going to in the previous scene, he's driving a Chevy Impala with plates 61795. I am not sure, but I think this error happened in the previous seasons as well. (00:09:08 - 00:09:48)
