Continuity mistake: While Johnny, Roy, and Vic try to outrun Charlene inside Stage 3, after heading down the ladder, when Roy opens the door to find Charlene standing there, Johnny's holding the red extinguisher in his right hand and the nozzle in his left hand, but next shot they've switched hands.
Continuity mistake: After leaving the football player who kicked in his TV screen, as Roy and John put the gear back in the squad, we see the old blue set of the Datascope 850 monitor and defibrillator, and not their current Datascope M/D2 white case. The last time the blue two-piece unit is even seen in the squad is in 4x13, "Parade", and the first time the white unit is used is in 4x15, "Transition", and it's then used in all following episodes. This scene was a prior taping, that was inserted into this end of the season episode.
Other mistake: When Dixie and Mr. Hart are on the elevator, we see the number five lit up on the panel above the door as they head down to the first floor in the ER, where they meet Johnny and the kids, who get on the elevator to head up to the fourth floor. The problem is that the ER is only a 2-storey building.
Transition - S4-E15
Visible crew/equipment: When Dixie and Carol stand in the ER hallway, talking about the three accident victims coming in and being swamped, there are two tape marks on the floor near their feet, where Carol and the other nurse were standing just before Dix walked over.
Camera Bug - S4-E10
Revealing mistake: When they wheel the stretcher with the unconscious teenager from the school fire into treatment 3, the stretcher is pushed up against the ER's gurney, and when the teen's right arm gets caught between the stretcher and gurney, the supposedly unconscious teen moves his arm up out of the way. Then he lifts his left hand when the attendant removes the blanket.
Daisy's Pick Blind Date - S4-E7
Visible crew/equipment: After rescuing the injured man from the burning theater, when Roy and John are on the roof getting the stokes with the victim onto 36's snorkel, we can see a crew member's arm/hand, wearing a white patterned long sleeve shirt, standing at the right side of the screen.
Visible crew/equipment: At the apartment complex fire, after Roy removes the ladder from Engine 51, the Pasadena engine rolls up, and when the Pasadena Captain walks toward Captain Stanley, the second cameraman wearing a brown blazer can be seen standing on the very top of Engine 51 to the left, shooting the overhead footage of the Pasadena engine that was just shown.
Visible crew/equipment: When the guys wake up in the morning and walk into the kitchen, the clear reflection of the camera dolly is visible on the oven glass door as it trucks to the right, when the guys see the empty mouse trap.
Visible crew/equipment: When Johnny and Roy are on their way to the parade in The Old Engine, in close-ups facing them, the reflection of a crew member's hand holding camera equipment is visible on the hood of the engine.
Factual error: Roy uses the defibrillator paddles on the electrocuted man twice, then after each defib Roy lifts the paddles off the patient's chest and they look at the EKG monitor, which shows flatlining the first time and a heartbeat the second time. Problem is, it's impossible for the EKG monitor to show anything at all. Either the defib paddles must be in contact with patient's body for the “quick-look” to determine the rhythm, which they were not, or the ECG electrode discs must be on his chest connecting him to the EKG monitor, and they were not.
Visible crew/equipment: When Station 51 is on scene at the vehicle accident in the canyon, after Marco uses a crowbar to free the driver of the station wagon and he pulls the driver out, a crew member standing behind the FX 6-ft wind machine fan (to blow smoke) are visible in the background beside the other overturned car. As an aside, in the previous and following shots that crew member and wind machine fan are in the same spot where Chet and Johnny supposedly are.
Visible crew/equipment: When Johnny jumps from the third floor ledge onto the lifenet, in the shot of his actual jump we can see a cameraman wearing jeans at the bottom left corner of the screen, who's holding a handheld camera shooting footage while lying on the street. The angle of that footage can be seen just as Captain Stanley shouts, "Gage, you first," and also when they walk the lifenet over to their left, before Cap shouts, "Okay, Roy!"
It's How You Play the Game - S4-E19
Visible crew/equipment: After the man with the herniated lumbar disc is removed from the waterbed and loaded onto the ambulance, when John gets into the squad, the reflection of a crew member is visible in the passenger side wing mirror.
Visible crew/equipment: When Roy drives off in the squad just as Animal Control shows up at the store with the tiger, we can see multiple reflections of array lighting on the vehicle and also the store's windows across the street.
Continuity mistake: When the Animal Control guys are dispatched to the fire in the canyon, the rearview mirror in their vehicle vanishes in the close-ups.
Continuity mistake: When Roy and John are talking to Vic at his party, his manager's cigar changes length repeatedly, from short to long and back again, depending on the camera angle.

Revealing mistake: When Roy's in the ambulance with the man who was rescued from under The Boat, he's on the Biophone with Rampart, and just after Roy says, "So we've occluded the arterial flow in the right arm," in the next shot from inside the ambulance, we can see that they pass a couple of parked GlamourTrams. The ambulance drives around Universal's lot while shooting the footage inside the moving ambulance, en route to Rampart.
Smoke Eater - S4-E16
Continuity mistake: When John and Roy are with the possible MI, while Roy's setting up the IV bag John's on the Biophone with Rampart, and after Brackett orders the IV TKO with MS, we see that Roy's pant leg has nothing on it, but then it cuts to the closeup and suddenly there are strips of torn tape (for IV as usual) on Roy's pant leg.
Smoke Eater - S4-E16
Visible crew/equipment: When Station 51 is dispatched to the 'structure fire', in the shot facing the squad as it starts to roll out, we can see the black tape covering the lightbar's chrome surface atop the squad, which also vanishes at the incident.
Answer: On IMDB, actress Anne Schedeen is listed as playing Nurse Carol on six episodes from 1974-76.
