Continuity mistake: Both Sam and Dean show no signs of bodily damage or harm, even though in the previous episode "Shadow," both brothers were attacked and their faces had been slashed by the shadow demons summoned by Meg.
Suggested correction: First part of the episode, it said "2 months ago" below the place where the episode takes place. So it probably took place before episode 16.
Thats before they showed up when the first group of kids went on the house and found the girl hanging.

Visible crew/equipment: Right before they light the house on fire, the shot cuts the to the hell hound guys and a cameraman is running up to them on the right. Not just an arm - his whole body, including the camera.

Audio problem: When Sarah goes to touch Evelyn's shoulder, you hear Sam's voice telling her to stop, except his mouth isn't moving at the time the phrase is being uttered. (00:23:50)

Other mistake: In the scene where Sam, Dean and Sarah go to compare the painting to the earlier photo of it, Dean, noticing the changed position of the razor, says, "Jared, check it out." He is speaking to the character Sam. The actor's name is Jared. [Confirmed in cast interviews].
Continuity mistake: As Sam is looking for the next victim in his visions, she walks by with her baby in a stroller. The baby is a completely different looking doll the first two times shown, but is blue-eyed Rosie when Sam approaches and interacts with her. (00:12:30)
Visible crew/equipment: When Sam and Dean are talking to Bobby about to leave after exorcising Meg, in the bottom right corner of the screen a cameraman's arm and camera can be seen. (00:17:10)
