Day One - S1-E2

Continuity mistake: In the opening scene, Gwen has a long, loose strand of hair hanging over her left temple - but every time the camera angle changes, it disappears. (00:01:00)

Ghost Machine - S1-E3

Revealing mistake: From the first "front on" view of Gwen and Owen in the arcade as they are running to catch the alien device, you can see behind Gwen to the right that Owen is being played by a stunt double.

Countrycide - S1-E6

Continuity mistake: In the pre credits sequence, when the lady gets out of her car the antagonist slashes her rear tyre however after the antagonist is attacking her inside the car we see a rear shot of the vehicle, and the tyre is fully inflated.

Greeks Bearing Gifts - S1-E7

Continuity mistake: While she's talking to Mary in the flat, Toshiko's hair keeps changing between shots, going from combed evenly to split apart with one section hanging in the middle of her forehead and the rest brushed back. (00:31:30)

Out Of Time - S1-E10

Continuity mistake: John is about to eat liver and potato with Emma before she storms out saying, "I don't like liver". While they are talking, the potato is facing Emma and Gwen. After Emma leaves, the liver faces Gwen and the spot where Emma was.

Combat - S1-E11

Factual error: Between getting disconnected by the bad guy and calling Ianto, Jack gets a dial tone on the dead man's cell phone. Cell phones don't generate dial tones. (00:11:10)

Captain Jack Harkness - S1-E12

Factual error: When Gwen finds Toshiko's hidden message from 1941, she realizes that it was written in blood, and her flashlight reveals the bright red letters on the now-yellowed piece of paper. But after 60+ years, the iron in the blood would oxidize. The writing on the note should be rust-brown, not red. (00:33:40)

Captain Jack Harkness - S1-E12

Factual error: "Captain" Jack Harkness actually wears the rank insignia of a group captain, equivalent to an army colonel and never abbreviated to "captain". In fact, the RAF does not have a rank of captain (the equivalent rank is flight lieutenant). Neither does it have the position of "squadron captain", as he is described - a squadron commander in World War II usually held the rank of squadron leader (equivalent to an army major).

Suggested correction: "Captain" Jack Harkness actually wears the rank insignia of a group captain, equivalent to an army Brigadier. The equivalent rank of Captain in the RAF is Flying Officer.

Mark Andrew

Nope. Group Captain = Colonel (Air Commodore = Brigadier). Flight Lieutenant = Captain (Flying Officer = Lieutenant).

Necrothesp

Group Captain in the RAF does in fact equate to Captain in the Royal Navy. The Army equivalent being, as stated, Colonel.

To The Last Man - S2-E3

Continuity mistake: In the scene on the pier with Toshiko and Tommy, the soldier from WWI, Toshiko is holding her hand bag but drops it when he picks her up. Immediately, the next shot she is holding it again.

To The Last Man - S2-E3

Factual error: Owen resuscitates Tommy using a defibrillator. The heart monitor shows a flat line, indicating Tommy's heart is not beating. A defibrillator has no effect in this situation. This is a widespread error in many films and TV shows.

To The Last Man - S2-E3

Continuity mistake: Gwen views a transparency of two Torchwood members from 1918. The words "Torchwood" can be seen behind them. When Ianto takes the transparency and holds it up, the slide is different: it contains five people and the word "Torchwood" is above their heads.

Meat - S2-E4

Continuity mistake: While he's in the van with Jack, Rhys's seat belt shoulder strap vanishes and reappears multiple times as the shots change. (00:27:40)

Reset - S2-E6

Continuity mistake: When the man is being terrorised by the weevil, in close up shots, the weevil is about 6 inches from the man's face. However in the long shots, it is about 3ft away from him. (00:34:35)

Dead Man Walking - S2-E7

Factual error: Martha picks up a bone saw to start the autopsy on Owen. An autopsy should start with a Y incision from just below the arm pits to the sternum and then down the stomach to the pubic region - using a scalpel, not a bone saw.

Jeff Walker

Captain Jack Harkness: Captain Jack Harkness, note the stripes.
Captain John Hart: Captain John Hart, note the sarcasm.
Captain Jack Harkness: Hey, I worked my way up through the ranks.
Captain John Hart: I bet the ranks were very grateful.

Trivia: The hand preserved in a glass jar in the Torchwood HQ is the severed hand of Doctor Who which he lost in a duel during the Christmas special. The hand features quite prominently in episode 1 but can also be seen in the background in most episodes.

Answer: He might have requisitioned or stolen it from Torchwood London, or he might have been ready down below once it'd been severed, searching for "the right Doctor" and not knowing that the newly-regenerated Doctor was the one he was looking for.
