Revealing mistake: When Derek Shepherd tells Meredith about his wife cheating you see Meredith Grey standing by her car and you can hear and see the rain all around the trailer, while Derek Shepherd is sitting on his porch under an overhang. Before Meredith sits down you can see rain hitting the hood of her car. When Meredith sits next to him she is completely dry. When she goes back to her car the camera does a close up and she is still completely dry even though her car is covered in rain drops and you can see the rain in the background. (00:37:10 - 00:39:40)

Character mistake: Meredith states that she cannot remember the last time her and Derek kissed. Derek says that the last time was when they were still dating, but it was actually at the end of season two episode three, after Addison arrived in Seattle and Derek and Meredith had stopped seeing each other. Suggested correction: She is referring to the last time they kissed without any issues. The last time they were good.

Visible crew/equipment: When Izzie closes the door with blinds hanging on it, before going to sit on Denny's bedside, you can see a crewmember with their filming equipment reflected in the glass of the picture hanging behind her. (00:35:00)

Continuity mistake: After Meredith puts her hand on the bomb, everyone stands up. It shows Christina and Burke standing up looking at her, But in the following wideshot Dr. Burke is suddenly crouched down again.
Factual error: Izzie tells her patient, Cheyenne and her mother, Mrs. Ward that they live in the same town she grew up in - Chehalis, Washington, yet mispronounces it. She says CH-halis (like CHurch) when it is SH-halis like Chinook Salmon.

Continuity mistake: In the first overhead shot showing Izzy lying under the Christmas tree, you can see Meredith's hair to Izzy's left but then the the next shot is Meredith coming into the room and Izzy under the tree by herself. (00:41:05)

Audio problem: When Meredith and Marshall are talking, he calls her Dr. Grey and she corrects him to call her Meredith. When he replies saying "Meredith" his mouth doesn't move.

Continuity mistake: When Christina notices the bomb squad moving Meredith down the hallway with the gurney, she leaves the room and removes her scrub jacket and hands it to a nurse. When she is out in the hall and reaches them, she suddenly has the scrub jacket back on again. (00:18:50)

Continuity mistake: When Izzie starts kissing Alex in the closet, as he tells her to stop she rests her hand on the back of his head. In the next shot it is on his cheek. (00:09:45)
Continuity mistake: When Derek talks with Mr. Chue in the rain under their umbrellas, in one shot Mr. Chue takes a big puff on his cigar. But in the next shot his face is relaxed and it is clear he didn't inhale; and then he takes the same puff again instantly anyway. (00:19:20)
Continuity mistake: As Meredith tries to get in to see Christina, but Bailey walks over to stop her, Meredith is putting on a white surgical mask. In the frontal shot she ties the white straps of the mask into bows. But in the next shot from behind the straps hang loose and aren't in bows. (00:29:50)

Continuity mistake: At the beginning when Meredith is laying on her bed, her head is dead centre on her pillow. In the next closeup her head is right on the edge, almost falling off of it. (00:01:05)

Continuity mistake: When Christina tries to talk to Burke about their relationship, he ignores her and she walks over to the banister and leans her head on her left hand. But in the next shot her whole left arm is down on the banister rail. (00:36:35)

Visible crew/equipment: When Meredith is walking away from Thatcher's house, there is a crew member that is between the house and mailbox who slips into the shadows. (00:33:12)
Continuity mistake: In the scene between Preston Burke and Cristina Yang in the locker room, Dr. Burke walks in wearing a brown jacket. During the first shot the collar of his jacket is lying flat. After he starts talking, the collar is sticking up in the back. After he says "I know you," the shot zooms out and his collar is lying flat again. When he says "I know you" the second time, it is sticking up again until the scene ends. (00:27:30 - 00:28:30)
