Head Case - S3-E15

Visible crew/equipment: Just near the end, as Ducky faces the other M.E. in Autopsy with his guilt, the camera shows a person reflected in the silver wall (who would be standing beside Ducky). It's not Ducky, as the M.E. is looking at and talking to Ducky with a different angle. The person appears to be dressed in medical scrubs, but the M.E. and Ducky are the only two in the room.
