Continuity mistake: In the scene where Gibbs' phone breaks, and he tells McGee to "reboot" it, Tony is in the back, getting a phone out of the cabinet. He starts to give the phone to McGee. The next shot, the cabinet opens again, and Tony pulls out the phone again and gives it to McGee. (00:04:55)

Continuity mistake: When Gibbs leads Tony and Kate to MTAC, Tony extends his arm in a "ladies first" gesture to let Kate go ahead of him, then drops his arm to his side. In the next shot, his arm is extended again and he drops it a second time. (00:16:30)
Continuity mistake: Gibbs asks Abby what she has got for him. Abby lists all the stuff she has found and holds up the magazine with 2 hands. Camera cuts and now she is holding the magazine in one hand. (00:09:30)
Continuity mistake: When Tony and McGee drive off the rear window is half open. The camera shows the car driving away and the rear window is closed. (00:14:40)
Continuity mistake: When Tony and McGee are driving in the car near the start, Tony has a dark shirt on. As the car drives off his arm outside the window has a light coloured top on. (00:14:40)
Continuity mistake: When Gibbs is leaning on the desk talking about the beetle, he has his legs apart. Camera cuts and they are crossed. (00:17:20)

Continuity mistake: The amount of sandwich Tony has eaten when Abby is being hacked is constantly changing instantly. (00:13:00)
Continuity mistake: Just after Fornell is arrested, the agents are showing NCIS their surveillance of the mafia. When doing so, the headset on the one agent changes position instantly. (00:22:00)
Continuity mistake: When Gibbs has the Mafia boss against the car, his arm is across the Mafia guy's chest. Camera cuts and now it is right next to his neck. (00:34:50)
Continuity mistake: When Gibbs is walking away from the FBI agent at the start, he is approaching a tree. As the camera changes he is way past the tree. (00:04:50)
Continuity mistake: When Ducky is doing the autopsy on the dog, he talks to the dog and leans against the slab on his elbows (the elbows are not seen however the motion is easy to distinguish). Camera cuts and his arms are wide and he's resting on his hands. (00:07:50)
Continuity mistake: When Gibbs catches McGee playing games on the MTAC system, McGee's microphone is down by his mouth. Camera cuts and it's up out of the way, instantly. (00:18:30)
Continuity mistake: When McGee turns up at the house, Dinozzo puts his hand on McGee's shoulder. Camera cuts and it's no longer there. (00:26:30)
Continuity mistake: When Gibbs is making the phone call to (presumably) the press, he is holding the receiver in his hand. Camera cuts and now his hands are on the desk. (00:31:50)
Continuity mistake: The security guard at the start goes to grab his gun and keeps his hand on his holstered weapon. When Gibbs gives the OK, he now has a handheld metal detector in his hand. In the shot showing Gibbs we can see the guard and he doesn't move. (00:00:35)
Continuity mistake: When DiNozzo is woken up, the cover falls off his shoulder. Camera cuts and it's back on. (00:24:25)
Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
