Character mistake: The uniform/costume that Major Tim Kerry, a Marine Military Aide to the President, is wearing, includes a Service Aiguillette made of 4 loops of round gold wire and scarlet cord. This aiguillette is worn by a Marine serving as an Aide or in other positions for a 4 Star General. A military aide to the President would wear 4 loops, all gold (as seen being worn by Commander Ray Trapp). (00:29:00)
Factual error: Special Agent Gibbs asks McGee how long Medical School takes in the UK, to which he answers 4 years. This is incorrect, because almost all aspiring doctors in the UK go through the Undergraduate Medicine route (MBBCh) which takes 5 years. When McGee performs a search on the computer, he enters "Medical Post Graduate" in the search criteria, which would be unlikely in real life. (00:24:27)
Visible crew/equipment: After Gibbs has Caitlin hydrate herself Caitlin asks the COB to show her to the Commander's facilities, and as Gibbs exits the Wardroom, the two T-marks are visible on the floor where Caitlin and the COB had been standing. (00:27:10)
Visible crew/equipment: When Agent Fornell follows Gibbs for a private chat (in the elevator) there's an overhead long shot of Caitlin, Tony, and Fornell's two agents. It then cuts to a shot facing Caitlin, and we can see the white mat taped to the floor with black tape (which was not in the previous shot), at the bottom of the screen. (00:22:50)
Visible crew/equipment: When Tony open the van's back door to show Abe's "daughter" the weapons they are alone, with Gibbs taking their photos from across the road. There's a closeup of the case of LAWs, and in the next shot a crew member (looking at the actors) is visible at the left side of the screen, standing directly behind them (she's in this shot only). Visible crew/equipment: When Caitlin refers to Gibbs as a Gloomy Gus it then cuts to Tony with Abby, and in the following shot facing the glass doors, the reflection of the camera and its operator are visible as they move toward the left of the screen. (00:14:05)
Visible crew/equipment: When it's discovered the M60s don't have firing pins Gibbs is pushed toward the green doors, and in the first closeup facing Gibbs the blue crash mat is visible at the bottom of the screen. (00:39:50)

Visible crew/equipment: During the joint interrogation of all four POS, in at least three shots facing the one-way mirror the reflections of the camera, the camera operator, and another crew member can be seen on that mirror. One example is when Gibbs tells Wong that the tape isn't his. (00:40:00)
Marine Down - S1-E9
Visible crew/equipment: After Gibbs meets with the real Colonel Walsh, when he's back at NCIS Gibbs asks for Morrow's help to gain access to records, and in the overhead shot a crew member's leg (wearing jeans and a sneaker) is visible on the left side of the screen, directly behind Morrow. Visible crew/equipment: When Nasser is brought to the interrogation room he raises his handcuffed arms and Gibbs nods no to removing them. In the next closeup of Nasser's handcuffs we can see the black taped mat on the floor (also visible in a following shot) which doesn't exist in overhead shots, then as the camera tilts up the reflection of the camera operator (wearing shorts) and camera are visible in the mirror. Visible crew/equipment: While Caitlin and Jane Doe are talking about blueberries it cuts to the doctor telling Gibbs that he can't release Jane Doe, and at the start of the next shot the pink T-mark is visible on the floor in front of Gibbs. (00:13:10)
Visible crew/equipment: While Gibbs and Caitlin are alone with Lt. Roberts trying to gain access to the Armed Forces Registry, just as Roberts acknowledges the critical 48 hours, the reflection of a crew member (presumably camera operator) is visible in the mirror, on the right side of the screen. Visible crew/equipment: After Abby tells Tony that he's "like a piercing, it takes awhile for the throbbing to stop..." as Gibbs asks her about the theory, the reflection of a crew member wearing a white short-sleeve shirt is visible on the glass. (00:19:40)

Visible crew/equipment: When Gibbs is talking to Kate about the "Harrison Ford movie" noting the terrorists were reporters, just as Gibbs mentions the Secret Service turncoat, the white T-mark is visible on the floor. (00:33:25)
Continuity mistake: When Nasser is brought to the interrogation room to meet with Agent Cassidy, at first the handcuff is around Nasser's left wrist with its single metal bar facing out, but when Cassidy says, "Now please," Nasser's handcuff is on the opposite way, with its double bar facing out. (00:35:50)
Character mistake: When Gibbs first looks at the victim's body and sees his rank, he incorrectly refers to him as a Lieutenant when the rank is for a Captain.
Continuity mistake: Towards the end, the news reporter's microphone is held with the WXEK facing forwards, then switches to have 19 at the front. (00:41:14)

Visible crew/equipment: When Kate is standing over the commander just before they take off, a red tape marker is visible between her feet. (00:09:45)
Visible crew/equipment: When Gibbs tells Abby to run the print through the military database a tape marker is visible by his feet. (00:09:40)
Continuity mistake: Duckey says there are 3 ways to kill a mongoose, while he has some form of medical instrument in his hand. However when he bends down to point at the victim's face, the instrument has disappeared. (00:04:10)
Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
