Continuity mistake: When Booth & Bones' taxi gets blown up & flips, the car lands upright, with nothing on the roof except the (now broken) taxi light. Yet when Booth squirms out the driver's side window, there are a few large chunks of asphalt sitting on the roof. (00:25:20)

Continuity mistake: When the taxi gets blown up we see the bag explode twice. Once which lifts the taxi slightly then a second time to help throw it over. (00:24:50)

Continuity mistake: When Booth drags Bones out the car, he places her on the sidewalk. While putting her down her position changes drastically between shots. (00:25:40)

Continuity mistake: In the Judge's chambers, Bones puts her hand up to ask if she can something. When the shot changes to a different angle, her arms are crossed in front of her. (00:37:50)

Continuity mistake: When Bones is dragged from the taxi after the explosion, she finds a tooth lodged in her arm (from bomb shrapnel). But she was pulled down for safety by Booth just before it went off, and was not exposed to the shrapnel - she was protected by the car door, and nothing was in a direct line to her. Added to that is the fact that her LEFT arm was hit - the arm that would have been on the 'inside/centre' of the front seat (the bomb went off on the right side of the car), and on which she was most likely lying - completely protecting that part of her body. Continuity mistake: Amy comes to see Brennan at night to tell her about her daughter's sickness. There are a bunch of letters on the kitchen counter. The envelopes move around in between shots, most noticeably the brown one. (00:03:20)
Answer: Answering my own question here but, having contacted various people involved in the production of Bones including Kathy Reichs, Hart Hanson, Eric Milligan, and Deirdre Lovegood who portrayed Taffet onscreen I finally have the answer. No-one know how she did it due to her constantly working alone. And she never revealed this information before her death.
