Continuity mistake: Series 5, Episode 18, "Here Kitty": When Judy (the patient) holds the tape out for House before her bronchial attack, the way she holds it changes instantly. (00:13:45)

Here Kitty - S5-E18
Continuity mistake: House spews cranberry juice all over the right side of Kutner's lab coat. In the next shot, the red stains have disappeared from the coat and are now on his shirt.
Other mistake: Amber states that Chase has a cat, even though in several other episodes Chase says he doesn't have a cat and wouldn't let Cameron get one because he hates cats. (00:16:10)
Continuity mistake: During the scene in which House is offering up liquor-flavored ice cream he sets the containers in a row down the table 1-2-3-4-5. He then puts spoons in the three containers closet to Foreman, 1-2-3. The shot changes and before anyone touches the spoons they are now in containers 1, 2, and 4.
Character mistake: When reviewing the results of the mercury testing on a patient, Dr. Taub says "Test results - Surprise, Surprise, Marcus Webly was wrong". The character to which Taub referred was actually Marcus Welby of 1969-1976 TV fame. (00:08:14)
The Tyrant - S6-E3
Continuity mistake: In the scene where the patient is having a heart attack, Dr Cameron is shown on the phone to House with dark hair despite having had blonde hair since season 4. She has blonde hair again in the following scene. (00:10:30 - 00:11:30)
Factual error: During the differential, Chase suggests the underlying condition is a systemic infection. House says this is unlikely since the patient received IV antibiotics the previous week. However, the doctor in the intro scene remarks that he thought the patient had C. Diff, which is treated with oral vancomycin, not IV. Oral vancomycin has next to no oral absorption, and would not treat a systemic infection. Moreover, even if vancomycin was orally absorbed, it would only treat infections caused by gram positive organisms and not other infections caused by gram negative organisms. (00:12:30)

Continuity mistake: Right after House has given the speech he and Wilson step outside and House removes his tie. After this his shirt collar changes between almost every shot, from being tucked in his jacket to being on the outside of it.
Character mistake: Episode 6.12, Moving the Chains: Marcus tells House that he and Eric were arrested for stealing a car, he was 16 and Eric was 14. He also says that his brother never did anything criminal after that. In season 1, we were told that Eric was 16 when he was arrested for stealing a car.
Character mistake: The girl who admits to changing Chase's profile says that what she is doing is negative reinforcement when it is actually positive punishment. Negative reinforcement involves taking something away (eg a noise) to encourage the behaviour whereas positive punishment involves adding something (the pictures) to discourage future behaviour.
Continuity mistake: At the start, Cuddy's arms are constantly changing position depending on the camera shot. (00:01:00)

Other mistake: When House is inserting foil pieces in the wall socket to create a short circuit in order to ignite a roll of toilet paper, he touches both pieces of foil simultaneously, which should have electrocuted him. (00:30:30)
Transplant - S8-E2
Character mistake: While talking to a patient awaiting a lung Transplant, Wilson comments that there is no urine in the catheter bag "despite taking in 200 cc of fluids." I suspect that Wilson meant to say 'despite taking in 2000 cc of fluids' as it would take more than 7 fluid ounces of intake (without noticeable output) before a doctor would begin to worry about decreasing kidney function. A minimum urine output of 30cc/hr is expected for a 70 kg human at a maintenance IV rate of 25-30 ml/kg/day (~73-88 cc/hr). (00:16:01)

Revealing mistake: When Wilson punches House in the face, is it painfully evident that he doesn't connect.
Charity Case - S8-E3
Continuity mistake: House is in the cafeteria eating chips with Adams discussing a case. Park walks up and gives Adams a cup of coffee. The cup of coffee disappears and reappears during the whole scene. The coffee is in front of Adams, then isn't. Towards the end the coffee is far to the side rather than where it originally was set. Continuity mistake: The runaway has an attack when her mother shows up and coughs blood on the table. There is an emesis basin at the far end of the table she throws up on. The shot changes twice and now there is FAR more blood on the table and the basin has moved to the middle of the table. Before the first shot change, Taub is reaching for the basin, however, after the change he is moving away without enough time to move the basin. The shot changes again and the patient is now in the bed with Adams holding the basin under her mouth. There is not enough time in that shot change for her to move from the foot of the bed to in the bed.
Factual error: When Dr. House is in the treatment room with a patient that has a gunshot wound to his foot, he removes a bloody dressing and treats the area without wearing gloves. No health professional would do this.
Revealing mistake: When Park is taking pictures of Adam's cleavage, the camera is turned off. The lens is retracted into the camera.
Answer: It comes from the phrase "Everything but the kitchen sink." He's saying that House has thought of "everything but the kitchen sink."
