Visible crew/equipment: When Leon is walking down the street, right before Crockett jumps in his car, the camera crew are visible in the shop windows the entire length of the scene, moving along with Leon.

Visible crew/equipment: In the scene where Crockett and Tubbs first meet at the deal, after the cops bust the deal too early, Tubbs steals Crockett's boat to get away. As he's driving the boat away, you see a second person kneeling down in the back of the boat, most likely a crew member. Very obvious.
Cool Runnin' - S1-E3
Continuity mistake: According to the ballistics report, the Jamaican hitters use "Interarms" (instead of Interdynamic) KG-99 submachine guns. However, the lead thug actually uses a Smith & Wesson M76, and his cohorts Jimmy & Winston use Star Z70s.
Visible crew/equipment: When Crockett and Tubbs are being escorted by boat to Bramlette's hideout by his wife Cassie, in a shot from the riverbank you see that Don Johnson and Philip Michael Thomas have been replaced by doubles. Although their backs are to the camera, it's plain to see that they look nothing like the actors who they're replacing. Especially Crockett's.
Other mistake: When Tubbs and Castillo are talking in the entrance way of the Senator hotel, at the end of their dialogue Philip Michael Thomas makes a very clear gesture with his right hand, which is Don Johnson's cue to finish his phone call in the background and approach the others.
Visible crew/equipment: At the beginning of the show where three guys take a ride in a convertible and one of them spits at a driver in a truck, pay close attention: the cameraman is visible in the reflection of the convertible, along with two crew members.
Continuity mistake: Zito's beard keeps disappearing and reappearing throughout this series due to the episodes not being shown in the order in which they were filmed.

Revealing mistake: After Crockett and Tubbs' confrontation with the New York police at the end of part 1, as the police car drives away, in the background a blue car drives by and a man and woman can be seen in the car looking and pointing at the filming.

Factual error: When Crockett takes out the hitmen in the nightclub, he fires 13 shots from his Detonics CombatMaster without reloading, when it has a maximum capacity of 6.
Continuity mistake: When Crockett tries to stop a rapist in a white Lamborghini the Lamborghini's taillights switches design during the chase.
Audio problem: When Gina shoots Nico Arroyo at the end of the episode, she fires twice but only one shot is heard.
Continuity mistake: When Clemente's messenger gets startled by Tubbs whilst talking to Crockett at the hotel pool, he pulls a Colt Python on them, which changes to a Smith & Wesson Model 19 with a bo-mar rib in a closeup.
Audio problem: When Charlie slaps Callie across her face, Charlie misses by several inches and the slap sound isn't heard until after Charlie completes the arc of the slap.
When Irish Eyes Are Crying - S3-E1
Visible crew/equipment: During Crockett & Tubbs' first meeting with arms dealer Max Klizer, a boom mic can be seen at the bottom of the screen in one shot.
Continuity mistake: Crockett and Tubbs are in his original black Daytona Ferrari. That car was destroyed in the first episode of season 3 and they received the white Ferrari Testarossa in episode 2.
Down for the Count (2) - S3-E13
Visible crew/equipment: When Crockett, Tubbs and Switek enter the mall at the end of the episode to prevent the hit on Guzman, Philip Michael Thomas has been replaced with an older looking stunt double who tries his best not to have his face in shot by immediately taking cover. In the next scenes in the mall, Tubbs has disappeared completely.
Continuity mistake: Many times during the episode, Det. Tubbs and the other characters mention that Tubbs killed Calderone. This is not accurate. In the episode "Caulderone's Return pt. 2," it is actually Crockett that kills Calderone and not Tubbs.
Revealing mistake: At the end when Tubbs shoots a man that is using his wife as a hostage, there is no muzzle or blast coming out of the gun.
Revealing mistake: When Bobby Diaz gets shot by Glenn McIntyre at the end of the episode, it causes him to fire his Beretta at the jetty he's standing on. Despite being at very close range, the deck remains unaffected by the gun shooting it. He also lacks any gunshot wounds.
