Continuity mistake: When John wakes up, the right-hand side of his head is covered in blood, both fresh and dried. When he's looking at Ecko's stick, his head has no blood on it. Fresh or dried. (00:02:40)
Continuity mistake: At the meeting, Jack's dad puts his hand on Jack's shoulder. When the shot changes, his hand has been removed. (00:27:40)
Continuity mistake: When Jack wakes up and hits the communicate button on the wall, his hand is below the box. When the shot changes, his hand is now to the side of the box. (00:06:00)

Continuity mistake: When Ben and his father climbs up the hill in the camper, there are obvious dents on the front-right and rear-right of the camper. But the camper looks brand-new when they stop on top of the hill. (00:35:50)
Left Behind - S3-E15
Character mistake: Kate's car is shown being towed, because, according to the tow-truck driver, "your fan belt is shot" The car in the scene is a 1988 to 1994 Buick Regal 2dr, which came with 2 engine options, a 3.1L and a 3.8l v6, both transversal engines. So, there could not be a "fan belt" on that car, as the pair of radiator fans on it were electrical. In any case, "your drive belt is shot" would have been the correct term to use, to describe the specific belt failure on that car. (00:02:19)
Factual error: The police officer introduces himself to Claire as "Officer" Barnes. In Australia, ordinary police officers hold the rank of Constable, not Officer. (00:06:10)
Through The Looking Glass (1) - S3-E22
Continuity mistake: On the plane, Jack's paper is on the armrests until he notices the article; at which point, it is now on the seat next to him. Pay attention to the black marker on the seat. The amount visible changes. (00:02:10)
Continuity mistake: Alex gets handed her gun while she has bloody hands. She then proceeds to run to her boyfriend in the woods. When she arrived, her hands are clean. (00:19:00)
The Man Behind the Curtain - S3-E20
Continuity mistake: When Sawyer gets back to the camp and is talking to Sayid, in the shots facing Sayid, Sawyer's hair is swept back but in the shots facing Sawyer it's hanging down his face. (00:09:30)
Continuity mistake: When Yemi is reaching out to the cross that Ecko is holding in the clearing, his hand/grip on the cross differs between camera angles. In one, for example, his pinky is curled up but out straight in the following shot. (00:36:30)
Factual error: The neurosurgeon introduces himself as "Doctor" Woodruff and also has this written on his coat. In Australia, where this scene is set, surgeons are always addressed and referred to as "Mister" after the British fashion.
Continuity mistake: When Sawyer dumps out the water canteen Juliet tosses him, his hand moves from the top of the base to the bottom, and the angle at which he pours the contents out changes, as well. (00:24:15)
Chosen answer: None of the main characters died in the plane crash. Many died on the island after the crash (Jack, Charlie, Sun, Jin, Daniel, Juliet, others), some lived on the island for an apparently long time after the crash (Hurley, Ben, Bernard, Rose) but some lived lives off the island after the crash (Sawyer, Kate, Claire, Miles, Aplert, and Lapidus). Remember that Christian tells Jack that "time has no meaning here," (in Purgatory). When everyone meets at the church, they are at the end of their lives however long that may have been and will now "move on" together. They look like they did on the island because that is the way they best remember each other.
