Ambush - S4-E1

Continuity mistake: Mark is intubating the man who is paralyzed from the neck down. We can see his hands working - we know it is him by the splint on his wrist and he is talking as he works. However, Malik comes in and calls him away, and he rushes off, but the hands working on the patient don't change.

Something New - S4-E2

Continuity mistake: When Jeanie and Al are in the diner, when she spears a piece of fruit on her fork, Al has his hand on his hip. However, when the shot changes and she says, "How about your grandmother?", his hand is up on her shoulder.

Something New - S4-E2

Continuity mistake: When Anna leaves the little boy Skyler's room, she has his chart under her right arm - but when she gets through the door and joins Doug in the hall, the chart has transferred to under her left arm, with no time for the change.

Continuity mistake: Oliver, the brain tumour patient, is talking to Anna, Henry, and Carter. He has his right arm up on the bed railing - but as he says "cook", his arm is suddenly down in his lap.

Good Touch, Bad Touch - S4-E5

Continuity mistake: When Carter is examining the patient Tom Angevine who drove into a tree, we can see two very noticeable parallel cuts from his eye to his chin - but as it goes to close up they miraculously disappear.

Continuity mistake: Near the start of the show, Peter is holding baby Reese upright when he opens the door to Carla's room, but as the shot changes to show him coming through the door, suddenly the baby is lying horizontally in his arms.

Ground Zero - S4-E6

Continuity mistake: Carter is talking to Carol about his grandmother. The shot is mainly on her but we can see him put his pen away and pick up a chart - then the shot goes to him and we see him do the same thing again.

Fathers and Sons - S4-E7

Continuity mistake: In season 2, Doug mentioned that he hadn't seen or spoken to his father in 22 years, since he was twelve. But when he's going through his father's things at the motel, he finds pictures of himself from high school. Plus, he mentions his father teaching him how to drive when he was about 13 or 14.

Freak Show - S4-E8

Continuity mistake: Peter, Elizabeth, and Romano are with the little boy who has organ reversal and as Romano says "genetic breakthroughs are in the wings", Peter puts on one glove. As Lizzie asks him if something is wrong, he is putting on the glove again. As they continue to talk, he begins to examine Rodney, but when the shot changes, he is still putting on his gloves.

Freak Show - S4-E8

Continuity mistake: When Peter comes in to talk to Rodney's father Isaac, the clock on the wall says 11:40 - which it had also read a few scenes earlier when Rodney came out of surgery. After Peter checks the chart and starts to talk to Isaac, the clock has somehow changed to 3:30.

Continuity mistake: Near the start of the show, Carter gets a can of soft drink from the machine and just then his cousin Chase shows up in the ER - they hug and somehow the can, which should be in Carter's hand, has disappeared.

Continuity mistake: When Carol goes to get Doug to see the sickle-cell patient, the clock reads 3:50. The next few scenes include Rachel arriving at the hospital and being taken to the dentist and for a dual makeovers by Cynthia. When Mark is walking by the same clock with Herb the lawyer, it reads 2:30. He then says goodbye to Herb and his ex-wife arrives to pick up Rachel, who is now back in the ER with Cynthia. The clock times do not reflect the sequence of scenes.

Continuity mistake: Just before Carter is let out of his jail cell, he is examining his cellmate's carbuncle - his patient had a cigarette behind his ear throughout the scene but as the shot goes to show the cop arriving, the cigarette has gone.

Continuity mistake: When Jeanie is in the office with Kerry, Anspaugh, and the union rep discussing her firing, her stethoscope changes position over her name tag several times within a few seconds.

Continuity mistake: After Dr. West leaves Anspaugh's office, the next shot shows both Kerry's hand and stethoscope have changed position. Also, the throw which had been fully visible behind her, is now only visible on the right side.

Continuity mistake: When Carter enters the hospital with the patient Lenny, he suddenly has latex gloves on, when just a second before, when the scene was outside, he had no gloves on. There just wouldn't have been time for him to get them on.

Continuity mistake: Within the space of what would appear to be more than 5 minutes in the ER, considering the continuous flow of action (starting with Kerry offering to get Pablo a taxi and ending with Dr. West examining the patient Lenny) the clock goes from 10:30 to 6:20. Shortly after that Dr. West pronounces Lenny dead, saying 13:38 (or 1:38).

Do You See What I See? - S4-E10

Continuity mistake: Even though Carter and Henry were outside in the snow when the patient Lenny arrived, and they had quite a bit of snow in their hair, when the shot goes to them entering the building, we don't see any snow on Carter and Henry - but there is still lots on the patient and the paramedics.

Love's Labor Lost - S1-E20

Continuity mistake: As Mark futilely pounds on Jodi O'Brien's chest, Carol can be seen in the background, removing her protective glasses with one hand. However, after the Time of Death has been called and they're showing the reactions of everyone present in the room (Susan, Carol, Carter, Lydia, Chen), Carol is seen taking the glasses off again, this time with both hands.

Answer: A man named Paul Sobriki comes into the ER in the season 6 episode 13 " All in the Family." He is complaining of headaches, Carter and Lucy decide to rule out meningitis and do a lumber puncture. Paul struggles during the procedure and believes that both doctors are stabbing him. Lucy, who is interested in Psych thinks something is wrong with their patient mentally, but Carter is dismissive of her. She gets cross with his attitude and she calls the psych department anyway to have a look at him. Later on and near the closing 20 minutes or so of the episode, all the staff start playing loud music and get a cake which appears to be blue lol, together to party as it's Valentines Day, Carter returns from somewhere and asks where Lucy is. He goes to check one of the rooms for her and Paul Sobriki comes up behind him in the dark and stabs him twice in the lower back, he collapses and as he falls unconscious he sees Lucy on the ground also. In the next episode, Be Still My Heart (ep 14), Dr Weaver discovers them and all of the staff work to try and save them, whilst Carter survives. Lucy's wounds are fatal and suffers a pulmonary embolism as Corday and Romano try to save her. Carter is grief stricken and feels guilty about the way he treated her, he becomes addicted to painkillers and Benton, Greene all help him and book him into rehab.

Answer: Lucy and Carter were stabbed by a patient she had suspected of having mental problems. Lucy died and Carter survived only to become addicted to painkillers.
