Visible crew/equipment: After the transport pod successfully ignites the moon, D'Argo and John are floating in space while Aeryn's in her Prowler, and just as we hear Pilot start to say, "The offspring Leviathan, Talyn, has broken from Moya's side," we can see the face of Rygel's puppeteer under the table, at the bottom right of the screen.
Visible crew/equipment: After Aeryn activates the cannon on the Leviathan baby's exterior, when Aeryn says, "Pilot...please let me know if any Peacekeeper scans are imminent," as she walks away the camera follows her, and we can see a nice reflection of the cameraman/crew member at the right side of the screen. The reflection's briefly visible again when Chiana says, "The only weapon we've got is that kid."

Visible crew/equipment: After the shot of Gilina, Aeryn, and John crouching under a floor grate, while hiding from the PKs, it cuts back to Moya, and just as Chiana reaches Rygel we can see the base of his hoverchair.

Continuity mistake: When Lishala gives John the map of the sky she asks him where his home world is, and John picks up a very small stone, but in the closeup he's holding a much larger stone in his hand, then in the wide shot it's small again before he throws it.
Visible crew/equipment: Near the start, while Rygel's annoying Zhaan and D'Argo by banging the noisy Y-rod against the Tadek board, when Rygel says, "It may be preferable to this mind-numbing tedium," the wavy dark hair of the puppeteer can be seen behind Rygel. Then later, when Zhaan says that she will strive to keep Kcrackic distracted, as Rygel's smoking the hookah we can see the movement of the puppeteer's arm under Rygel's robe.

Later, after Zhaan tells Rygel to find Durka's corpse because it'll set him free, when Rygel's back aboard the Zelbinion, although he's just remembering being in a cage in front of Durka, it's interesting to note that we can see the puppeteer's movement behind Rygel.

Revealing mistake: While John and Aeryn are in the module, which is leaking plasma, just as Pilot tells them that Moya does not want to put her baby at risk, we can see the puppeteer rods controlling Pilot's arms as they move.
Continuity mistake: In the scene where Crichton and Crais are fighting with the chains in the room with the metal whirly-gig you see Crichton fall and Crais wraps the chain around Crichton's boot. In the very next shot you see Crais yank on the chain with all his might, but the chain is now attached to Crichton's hand instead of his boot.
Chosen answer: It's never explained. John mentions a theory that his less technologically developed craft simply interferes with the wormhole less than the "superior" Vipers, but they never make a concrete determination on that point. But we also have to take into account the fact that Moya traverses wormholes with no ill effect as well.
