Character mistake: Horatio twice reads an extract from a letter threatening the life of the poisoned woman. The first time, he reads, "If you ever try to contact us again we will kill you," and the second time, "If you ever try to contact me again I will kill you." A close up of the letter shows it to read, "If you ever try to contact me again i'll (sic) kill you".

Factual error: During their investigation, the CSIs learn, through a number of phone calls she made to her mother, that their murder victim was in the Witness Protection Program. Contacting family members is one of the cardinal sins of WITSEC; she would have been immediately relocated with another new identity, and not left in a city where people who want her dead know where she is.

Continuity mistake: When Jeff Corwin is explaining to Calleigh and Delko that the tooth they have came from a crocodile instead of an alligator, the jar in Delko's hand very quickly jumps from one hand to the other. (00:25:50)

Plot hole: There is absolutely nothing that would give Horatio any reason to think the severed arm they found was from a previous victim of their suspect. It doesn't make any sense he would pick something as random as the arm that has no connection (other than it being found during the search for the girl) to the case.
Factual error: One of the police officers makes a comment about an unregistered firearm. Firearms don't need to be registered in Florida.
Continuity mistake: At the beginning the media broadcasts reports about hurricane Andrew, but later into the episode, this changes to Hurricane Anthony as the episode is titled.
Factual error: The stock footage of the bull shark is actually of a Great White.
Revealing mistake: There is a dead (burnt) woman in the morgue. At the end of the episode, Alexx pulls her out of her vault for one last look. When you look at her neck, you will see a pulse. (00:42:30)
Stalkerazzi - S2-E15
Plot hole: The photographer who found the dead guy (Stalkerazzi) told Horatio that he was chasing a black Cadillac Escalade that was chasing the Stalkerazzi's car. He also said that he got stuck behind a truck and that when he finally caught up the Stalkerazzi was dead and there was no sign of the black Escalade. Throughout the rest of the episode nobody does any looking into the black Escalade, asks suspects if they own one or even mention it again. No investigation of the truck that the photographer got stuck behind was shown either. In the same episode Speed tells a cop, while in a tree looking into someone's yard, that they go in through windows if need be to close a case. This is totally inconsistent with the rest of the show and the law. They are always careful about getting search warrants and doing things By the Book.
Character mistake: While discussing the death penalty with Mac, Horatio observes that New York hasn't executed anyone since 1976. The last time New York executed an inmate was actually thirteen years earlier, in 1963.
Continuity mistake: After Dave Penrod jumps from the rooftop to escape from Horatio and Mac, the pair reach the ledge far too quickly from where they were previously, at least a building away.
Factual error: In the scene in the lab where they notice the porn actress's wisdom teeth have not erupted yet, they show her lower jaw with the wisdom tooth just under the gum, but say it is 5 cm under the gum. I don't know about you but if my wisdom tooth was 5 centimetres under the gum, it would be hanging out the bottom of my jaw. (FYI, 5 cm is almost 2 inches.)
Plot hole: Alex determines the actress is only about 18 because her wisdom teeth haven't erupted yet. The problem is they don't always erupt, I'm 32 yrs old and mine have never erupted.
Factual error: As the funeral procession is going to bury Speedle, they pass by Pacific National Bank, which is not in Florida, but in California where the series is filmed. (00:40:00)
Factual error: Error takes place in the last few minutes of the episode when Medical Examiner Dr. Alexx Woods responds to the scene of a DUI with two deaths. The Miami-Dade County officer on scene tells her that she may know one of the victims, since the victim had a M.E. pass on the rearview. You can see that his radio, which we assume is department issue, has the logo on the lapel microphone of "SJM Industrial Radio" a radio rental and service shop based out of El Segundo, CA. Why would a police department in Florida rent radios from a radio shop in California? Guess they overlooked the logo when they decided to rent radios locally since the majority of all outdoor scenes are filmed in Long Beach, California, as well as portions of Manhattan Beach and Redondo Beach.
Character mistake: When Calleigh is with the medical examiner, she says that "Togo" is holding down the scene at the hospital, stating the actor's actual last name instead of Wolfe, his character name. (00:08:25)
Other mistake: When Valera hands Ryan the results from her comparison of the nanny and the baby the results list XY (male) for the sex chromosomes instead of XX (female). (00:32:30)
Continuity mistake: When Wolfe is outside holding the baby, the hood on Bethany's jacket is off her head, then back on, then off again, without him ever touching it.
Answer: Earlier in the episode, Horatio tells Mac that Penrod was seen in Tribeca getting his leg set, only to bolt when he learned a nurse wised up the cops. Caine and the NYPD probably got Penrod's address from the hospital.
