Deliberate mistake: In episode three, the Doctor and Vira meet a half-changed Noah. He holds a gun on them, then drops it at Vira's feet and the door is suddenly closed. This is because the scene was originally longer and involved Noah begging Vira to kill him before the Doctor shuts the door. It was trimmed down to get it past BBC censors.
Genesis of the Daleks - S12-E4
Visible crew/equipment: In episode 1, when General Ravon is being held at gunpoint and he says 'they won't last much longer', the shadow of the boom pole is moving on the door behind him. (00:15:15)
Revenge of the Cybermen - S12-E5
Continuity mistake: At the end of "Genesis of the Daleks", the Doctor was wearing his big brown overcoat. However, at the beginning of this episode, he is wearing his red jacket again.
Deliberate mistake: Benton is a Warrant Officer in this story, but the end credits refer to him as "Sergeant Benton".
Revenge of the Cybermen - S12-E5
Revealing mistake: At the climax of episode four, the Beacon plunges close to Voga and orbits very fast before the Doctor manages to break free. The planetscape that appears on the screen is quite obviously a revolving drum with bits of clay on it.
Genesis of the Daleks - S12-E4
Visible crew/equipment: Just after The Daleks move past the Doctor and the female Thal soldier in the corridor, a piece of lighting equipment is reflected on the wall behind her. (00:08:00)
Revealing mistake: When Noah bangs his green arm down, in a close-up you can tell that the arm has green bubble wrap around it. (00:02:30)
Revenge of the Cybermen - S12-E5
Plot hole: Sarah is transmatted down to Voga after being injected with alien poison by a Cybermat. According to the Doctor, the transmat will recognise Sarah's human molecules and separate and reject the (non-human) poison molecules. As clothing fabrics tend to made of non-human molecules, how come Sarah and Harry didn't arrive on Voga stark naked?
Genesis of the Daleks - S12-E4
Continuity mistake: When the Doctor drops the time ring, it lands very close to the wall. When he picks it up later, however, it's in the middle of the corridor. (00:09:40)
Revenge of the Cybermen - S12-E5
Other mistake: Shortly after the Doctor makes his speech to the Cybermen about how pathetic they are, you can see a Cyberman's head wobble.
Revenge of the Cybermen - S12-E5
Continuity mistake: For a supposedly emotionless creature, the Cyberleader is easily provoked to a fit of rage, including throwing the Doctor around the Beacon.
Revealing mistake: In the last part, the giant robot steps on a soldier. There are several things wrong with this scene. 1) You can see the matte halo around the robot's foot. 2) The robot does not leave a footprint. And 3), after the soldier gets squashed, his arm is visible waving over the robot's foot.
The Sontaran Experiment - S12-E3
Continuity mistake: In Episode 2, when the Doctor calls Harry from the Control Room and Harry enters the room next to the Cryogenic Chamber, there is an oblong yellow panel next to the door. It extends from about Harry's waist line to about 200mm below the top of the door. But in Episode 3, when Noah calls Vira from the control room part way into his transformation, and everyone runs into the adjacent room, the panel is mounted differently. It now extends from about Harry's mid-torso, to the top of the door frame. (00:03:35 - 00:09:32)
Answer: TARDISes are generally available for properly authorised use on Gallifrey; they're not usually assigned to a particular Timelord on a long-term basis. The Doctor stole his when he left his homeworld.
