Revealing mistake: In episode 3, the section of wall K-9 knocks down is obviously precut, complete with debris on the floor.
Earthshock - S19-E6
Revealing mistake: During the "cliffhanger ending" at the end of episode 3, it's blatantly obvious that the three columns of advancing Cybermen are merely mirror images of each other. The head of each column has black head bars indicating leadership, and is holding his gun in exactly the same position.
The Creature from the Pit - S17-E3
Revealing mistake: Just as the Doctor asks where the TARDIS has materialised, watch the control console wobble. However, isn't it strange that the hat-stand in the TARDIS console room remains upright thoughout...so why does the console wobble, and the hat stand not move in the slightest?
Revealing mistake: Immediately after the Doctor stabs SV7 in the back of the head and the robot crashes to the floor, the next scene shows the fallen figure of SV7 in the background - breathing heavily, with his chest moving up and down.
Revealing mistake: The split screen used to make two Doctors appear simultaneously is apparent when the Captain's chair appears to blur while both are in shot.
Revealing mistake: When Benton jumps over the maggots to get into the cave the Professor and Jo are hiding in, the jump causes the floor to move, giving away the fact that the floor is wood covered by a small amount of gravel.
The Talons of Weng-Chiang - S14-E6
Revealing mistake: There are *modern* power points, covered with masking tape, visible on the walls of Litefoot's laboratory. The story is set in the 1890s.
Revealing mistake: Near the end of episode one, the Doctor peers into a hopper and sees ore piling up inside. The image of the ore wobbles back and forth.
Revealing mistake: In episode four, when Vishinsky and Sarah go into the storage room to get the force field generators, the plaque on the door flops around like the cardboard that it is.
The Trial of a Time Lord 13-14 (aka The Ultimate Foe) - S23-E4
Revealing mistake: In the final episode, Glitz and the Master leave the console room while the Doctor is driven into submission, but they are quite obviously standing by the scanner screen.
Revealing mistake: The Grove is one of the most obvious indoor sets masquerading as a lush outdoor environment. The sound the actors' feet make on the floor is especially unforgivable.
Revealing mistake: At the beginning of episode four, when the Doctor is surrounded by robots, we initially see SV7 with *blue* eyes. Clearly someone forgot to add the glowing red-eye CSO.
Revealing mistake: When Stahlman is killed - the second time - the shot of his green primord face reveals the actor's unpainted neck under his beard.
Genesis of the Daleks - S12-E4
Revealing mistake: When Ronson is killed by The Daleks, you can see Ronson reacting a little too soon to the Dalek laser. (00:14:40)
Genesis of the Daleks - S12-E4
Revealing mistake: In episode 6, when the Thals detonate the charges, smoke is seen coming through the door before the detonator is pressed down. (00:19:40)
Earthshock - S19-E6
Revealing mistake: When Lieutenant Scott kills the second Cyberman on the bridge, he starts falling down way too early.
Invasion of the Dinosaurs - S11-E2
Revealing mistake: When Grover and Whitaker struggle over the time machine at the very end of episode six, the console wobbles and jerks as though it were very lightweight.
Earthshock - S19-E6
Revealing mistake: The "dead" crewmembers at the end of episode two can be seen breathing.
Revealing mistake: In Episode 3, when Dask is cutting the wires, the second explosion happens too early.
The Caves of Androzani - S21-E6
Revealing mistake: When Peri falls into the spectrox nest, watch for her to reappear as she bounces off a mattress.
Answer: It was never destroyed on-screen; it was intact at the end of the TV movie, and destroyed by the start of the 2005 series. It was destroyed in the novel "The Ancestor Cell," but in a completely different manner to what happened in the series.
