The Power of the Daleks - S4-E3
Revealing mistake: The Daleks that appear in this and other Doctor Who stories were rather expensive, so some ingenuity was required to make the small number available look like they were more than they were. Tricks employed in "The Power of the Daleks" included making four Daleks look like an army by having them circle around the set a few times (note the delay between the fourth Dalek exiting and the "fifth" Dalek entering), and the blatant use of photographic blow-ups in place of "real" Daleks.
The Tomb of the Cybermen - S5-E1
Revealing mistake: During the fight scene between Toberman and the Cybercontroller, the Cybercontroller uses a very visible harness to lift Toberman over his head, and the wires attached to the harness Toberman is wearing are also very visible. When Toberman, in return, lifts the Cybercontroller above HIS head, it is obvious that what Toberman is lifting is an empty costume, with no-one inside it.
Revealing mistake: When the robotic Doctor first appears - and in many of the later scenes involving the jungle on Mechanus - it is very obviously not William Hartnell playing him, but a vaguely similar double. In addition, his lines have been dubbed by Hartnell and do not always match the movements of his mouth.
Planet of the Spiders - S11-E5
Revealing mistake: In episode three, when Tommy peers into the room where the Doctor and Sarah are talking to Cho-Je, the camera moves forward - and bumps into a wall with an audible thud and noticeable jerk.
Genesis of the Daleks - S12-E4
Revealing mistake: Whenever there's a close-up of Davros, it's very easy to see that there's a gap around his mouth where the mask should blend in with the actor's face, but doesn't. This mistake was rectified, incidentally, in Davros' next appearance in "Destiny of the Daleks."
The Dalek Invasion of Earth - S2-E2
Revealing mistake: When a rebel throws a bomb from the second floor of the Dalek saucer, notice it explodes too early and in the wrong place.
Revealing mistake: When the Doctor falls out of the TARDIS, near the beginning, he nearly manages to pull off the entire TARDIS door.
Revealing mistake: The explosion of Irongron's castle is obviously stock footage of a quarry explosion.
Revealing mistake: As the cast move through the transom of the space station, observe the starscape through the lower windows. At certain points you can see a thin strip of light where the black curtain used for the stellar backdrop meets the studio floor.
Revealing mistake: In episode one, Scaroth's 'Jagaroth skin' on his wrist flaps about. Either that's a badly fitting piece of costume...or he's suffering from severely peeling skin.
Revealing mistake: At various times (especially in episodes 3 and 4) you can see actor Stephen Thorne's lips through Omega's mask (and in one profile shot you can also see his jaw), yet when his mask is removed, we find that Omega has been dissipated and has no face...
The Moonbase - S4-E6
Revealing mistake: The doors and walls wobble badly as Jamie and Ben barricade the entrance to the medical unit.
Revealing mistake: When the Doctor taps the ice crystals, they wobble instead of breaking.
The Monster of Peladon - S11-E4
Revealing mistake: At the end of episode 2 / beginning of episode 3, the Doctor and Sarah are thrown into a pit. It is quite obvious that they hit a flexible mat when they land. it is gone when they stand up.
The Rescue - S2-E3
Revealing mistake: At the beginning, as the Doctor is standing in the TARDIS door, the cave wall can be seen through the TARDIS.
Delta and the Bannermen - S24-E3
Revealing mistake: The bodies lying on the ground during the initial battle are rather too obviously rubber dummies rather than real people.
Revealing mistake: In the very first shots of this Doctor Who story, it is obvious that the "Empress" Spaceliner is a model - you can even see the stick that holds the model in place.
Revealing mistake: In episode 5, Codal discovers that what he thought was Taron is an invisible Spiridon. However, there is a glimpse of the actor inside the fur costume as he attacks Codal.
The Creature from the Pit - S17-E3
Revealing mistake: Before the TARDIS console explodes (as Erato is weaving his shell around the neutron star), Tom Baker casts a sharp look over his right shoulder...no doubt looking to see if the ground was clear where he was due to fall.
Revealing mistake: When the Doctor confronts D84, the robot is shown in profile as the Doctor forces it to speak. The movement of the actor's jaw is visible behind the mask.
Answer: In 'The Five Doctors', three separate Cyberleaders are definitely used. So it's likely that Cyberleaders are like unit commanders, of which a fair-sized army might have several.
