The Horns of Nimon - S17-E5

Continuity mistake: In episode 4, approximately 5 minutes from the end of the story, Soldeed is shot after he pulls the lever that sets the complex on overload, and then crumples to the ground dead beneath the lever. Then, after the Doctor and everyone else get out of the complex, we see a selection of shots taken inside the complex - one of which is the room with the lever that Soldeed pulled, except he's no longer there...

Nightmare of Eden - S17-E4

Continuity mistake: When the Doctor finds the vraxoin vial in episode 1, he puts it in his right coat pocket, from where it is then removed by Stott. However, at the end of episode 2, Costa's scanner finds vraxoin traces in the Doctor's LEFT coat pocket.

The Three Doctors - S10-E1

Continuity mistake: (End of Ep. 3 and start of Ep. 4) When The Third Doctor fights the dark side of Omega's mind, the rings that The Doctor wears on the pinky fingers of both hands switch to his ring fingers, then back again to his pinky fingers, between shots. Then as Omega chokes The Doctor, the ring on his right pinky vanishes between shots.

Continuity mistake: The Doctor somehow manages to secure his scarf across the catwalk, despite having only gone on seconds before the Master. He then manages to put it on again during the seconds it takes for the Master to alter the angle.

Continuity mistake: In the opening TARDIS scene, after the TARDIS lurches from the time scanner, the Doctor's hat rack can be seen standing upright. Then Leela picks it up in the next scene.

The Twin Dilemma - S21-E7

Continuity mistake: When the twins start to play equations they are both facing towards us. If they were to face each other, their legs would be touching each other. During the game we again see them at this distance apart. But the third time we see them (as they finish), they have moved at least a metre apart, making it possible for them to face each other comfortably.

Continuity mistake: After Skagra demands the book from Professor Chronotis, as Skagra's sphere floats toward Chronotis he's wearing his eyeglasses, but then the Professor's eyeglasses vanish and reappear while his hands are raised.

Continuity mistake: During the animated portions of Shada's missing scenes, in numerous shots an illustrated character's clothing (buttons, scarf, etc) become flipped, sometimes even within the same shot. Example: when The Doctor goes to the physics lab and meets Clare Keightley, note her buttons and The Doctor's scarf.

Continuity mistake: The Doctor removes his coat and it's left in the prison cell but in the brief shot of the Doctor and Barbara pushing Dalek Ian, the Doctor is inexplicably wearing his coat again.

The Evil of the Daleks - S4-E9

Continuity mistake: (Animated version). Waterfield puts down the photo of the Doctor on his desk but the next time we see his desk the photo is no longer there and then later it's back on the desk again, without Waterfield having moved it.

Survival - S26-E4

Continuity mistake: At the beginning of episode 3 when the Doctor tells Ace that she can do what the Cheetah People can, he puts his hand on her shoulder. In the next shot his hand is no longer there, but it's back again in the next.

Continuity mistake: When Karra dies and turns back into her human/humanoid form she is wearing a black top that they did not wear as a Cheetah Person.

Planet of the Spiders - S11-E5

Plot hole: At the climax of episode 2, the Doctor is about to catch Lupton when the latter simply teleports to safety. So why didn't he do that in the first place, before engaging in a 15-minute chase?

Trivia: This Doctor Who story was originally scripted and produced as a four-episode story, but, just two weeks before transmission, upon viewing the story, co-creators Sydney Newman and Donald Wilson felt that the final two episodes (Episode 3, 'Crisis'; and Episode 4, 'The Urge to Live') should be combined into a single episode. The new 'condensed' episode incorporated the opening titles of 'Crisis' with the closing credits of 'The Urge to Live'.

Answer: It was never destroyed on-screen; it was intact at the end of the TV movie, and destroyed by the start of the 2005 series. It was destroyed in the novel "The Ancestor Cell," but in a completely different manner to what happened in the series.
