Continuity mistake: In this episode part of the TARDIS set was erected incorrectly, resulting in some of the roundels emerging from the wall rather than being indented into it. This is very visible in all the TARDIS 'console room' shots.
Continuity mistake: In episode 4, approximately 5 minutes from the end of the story, Soldeed is shot after he pulls the lever that sets the complex on overload, and then crumples to the ground dead beneath the lever. Then, after the Doctor and everyone else get out of the complex, we see a selection of shots taken inside the complex - one of which is the room with the lever that Soldeed pulled, except he's no longer there...
Continuity mistake: After Skagra demands the book from Professor Chronotis, as Skagra's sphere floats toward Chronotis he's wearing his eyeglasses, but then the Professor's eyeglasses vanish and reappear while his hands are raised.
Continuity mistake: During the animated portions of Shada's missing scenes, in numerous shots an illustrated character's clothing (buttons, scarf, etc) become flipped, sometimes even within the same shot. Example: when The Doctor goes to the physics lab and meets Clare Keightley, note her buttons and The Doctor's scarf.
Continuity mistake: In "The Deadly Assassin" the Doctor's blood is red but in "State of Decay" his blood is suddenly dark blue. Then it's back to being red in "The Two Doctors."
Continuity mistake: The Doctor somehow manages to secure his scarf across the catwalk, despite having only gone on seconds before the Master. He then manages to put it on again during the seconds it takes for the Master to alter the angle.

Continuity mistake: In episode four, as they approach the TARDIS, the female member of the remaining troopers is grabbed and doesn't make it inside. But when the troopers do get inside the TARDIS, it's a man that's missing.

Continuity mistake: The mouth panel of the Cybermen costumes was originally intended to be a clear perspex panel. During filming, it was felt that this showed too much of the actor inside the Cyberman costumes. So a decision was taking - mid way though filming - to paint over the 'clear' panel with silver paint. However, this does result in the mouth panels of every Cyberman alternating from 'clear' to 'silver' from shot to shot.
Continuity mistake: After the Raston Warrior Robot has killed all the Cybermen, watch the dead Cyberman in the back right of the picture. His arm position changes as the robot teleports away.

Continuity mistake: When the Doctor and Peri first land on Androzani, mountains are seen in the background. They disappear when Peri picks up the silica. A couple of shots later, they're back. (This mistake was mentioned on the DVD commentary; the mountain effect was left out for financial reasons).

Continuity mistake: When the Doctor is climbing through the tunnel to the Queen bat, note the question mark on his lapel: the image is reversed.
Continuity mistake: When the twins start to play equations they are both facing towards us. If they were to face each other, their legs would be touching each other. During the game we again see them at this distance apart. But the third time we see them (as they finish), they have moved at least a metre apart, making it possible for them to face each other comfortably.
Continuity mistake: When the Doctor enters the desert room in addition to his coat, he is wearing a brown waistcoat with a white shirt underneath. When the shot cuts away and the back to the Doctor, we see him take off his coat but he is no longer wearing the waistcoat. Later when he collects his coat after the acid bath scene and he starts to look through the wardrobe for a disguise he is mysteriously wearing the waistcoat again.
Continuity mistake: The amount of dirt on the Doctor's face changes between the Bath House and the exterior filming.
The Trial of a Time Lord 5-8 (aka Mindwarp) - S23-E2
Continuity mistake: When Kiv wakes up in Peri's body, (s)he orders Crozier to kill the body Kiv was using before. Crozier responds that it's already dead, but in the previous shot the old body could be seen breathing.

Continuity mistake: The Doctor can be seen wearing glasses in some long shots of him riding a motor bike. Normally, Sylvester McCoy (who played the Doctor at this time) removed his glasses before filming. Seems as though he either forgot, or could not ride a motorbike without them.

Continuity mistake: Due to some sloppy editing of the "cliffhanger ending" at the end of episode 2, and again at the start of episode 3, the Doctor, Mel and Burton are seen to freeze in place for no apparent reason, just BEFORE we see the Bannermen line up and aim their guns at them.
Remembrance of the Daleks - S25-E1
Continuity mistake: After the Special Weapons Dalek blows up the gate to Ratcliffe's warehouse, you can see that some of The Daleks were slightly damaged by the explosion. In all the following shots, they have been repaired.
Answer: It was never destroyed on-screen; it was intact at the end of the TV movie, and destroyed by the start of the 2005 series. It was destroyed in the novel "The Ancestor Cell," but in a completely different manner to what happened in the series.
