Continuity mistake: End of the first half. Ryo hands a yellow towel to his right and a blue towel to his left. They are swapped around in the reverse shot, and Shingo Takasugi is standing with them too. (00:07:25)
Other mistake: If Meiwa scored already 10 goals the previous year (when the championship was won by Shuutetsu) it is hard to explain how nobody from Shuutetsu knew about Hyuga, since they were serious contenders. (00:13:40)
Other mistake: Manabu loses his glasses; Sanae hits him with her flag, but the action is animated incorrectly, and he goes down without the pole connecting. (00:09:10)

Factual error: It's the first leg of a tournament between elementary school players, and people skip work to see it, the city streets are empty because everyone is home watching it (since it is televised, too!)... That just never happens. It is not a world different from our own where everything is based on the sport (like Pokemon).
Continuity mistake: Takeshi Sawada dribbles 9 players, he has 1 left in front of him, but still he is facing Misaki too, and dribbles twice the same players (Urabe and Taki). (00:11:40)

Character mistake: Ishizaki fails to recognize that the 'giant' he met is a goalkeeper. He was wearing long sleeves, goalkeeper gloves and when he walked away Bruce kept staring at him...and the enormous number 1 on his back. It takes a very special kind of football player to not identify someone like that as a goalkeeper.

Other mistake: The whole team is supposed to be at the opening ceremony, but only 9 players are in the group. Ishizaki in the bathroom, but still there are people missing. (00:12:00)

Other mistake: When Genzo browses the coach notes and figures out instantly what should have been obvious to anyone (the opponent never conceded any goal, that is no small detail!), the way the action of the opponent is shown, both attackers were in offside. (00:11:55)

Other mistake: During the montage with the victorious march towards the semifinals, a Nankatsu supporter is waving a pure white flag instead of the team one. (00:11:00)

Other mistake: The perspective when Mr. Ozora tells Tsubasa "I'll let you go to Brazil" is wrong; it looks like he's standing right in front of him, but he's on the bleachers. (00:04:50)
Other mistake: The tryout match happens on a field with no audience at all. Hyuga just lies there on a bench without anyone saying anything to him. Both the coach and Misaki know who he is, but nobody noticed him when he was a hugely conspicuous presence. Of course his interference in the match is pure anime silliness; nobody bats an eyelid during his invasion, and he fetches the ball from the forward unnoticed, coming from the opposite goal area.

Other mistake: After Misaki's flashback, Tsubasa stops the football, that is animated with a weird flickering under his foot. (00:06:20)
