Continuity mistake: When Misugi slaps Yayoi in the face for unsettling his rival, Yayoi's cheek lights up instantly, but the bruise is gone immediately. (00:05:45)
Audio problem: In the Italian dub, starting with the match against Musashi FC ("Mambo FC"), Nankatsu's goalkeeper changes all of a sudden last name, from Alan Crocker to Alan Parker.
Other mistake: Sanada passes the ball to Honma as per Misugi's instructions. Honma gets it and runs...in the opposite direction, towards his own goal! Somehow it is the right direction, though. (00:13:20)
Continuity mistake: Throughout the various episodes set at the sport facility near Yomiuriland for the final phase of the elementary school tournament, the ferris wheel changes size and number of its carts, and sometimes coloring.
I Can't Play - S2-E7
Other mistake: Tsubasa is way behind Misaki and coming in at a slower speed, but somehow Misaki throws a deep ball to him - and Tsubasa almost catches it! It's impossible to do that in the way shown. (00:13:20)
Other mistake: All of a sudden and with no explanation, in this episode the coach and the cheerleading team have things like whistles and megaphones that they never brought to matches before and didn't have when the match was underway in the previous episodes. (00:05:55)
No Victory in Semi Final - S1-E21
Plot hole: The whole team went into the match, and the tournament, without knowing its rules. They all are totally surprised by the fact that the top 2 teams qualify from each group. Of course, that's quite impossible, especially since there are veterans in the team, Tsubasa is a football fanatic, Misaki is a clever student of the game, and you just don't walk into a tournament you have prepared for and looked forward to for ages without knowing such a basic fact.
No Victory in Semi Final - S1-E21
Continuity mistake: Misugi's goal happens with either a high ball that curves downward, or a ground-level ball that rises suddenly, depending on the camera angle. (00:20:10)
Continuity mistake: Roberto realises Hyuga's fright technique; as he does that, the person between himself and Mrs. Oozora is wearing a white shirt. Reaction shot on her, and it's a man in yellow that was in a different position earlier. (00:11:20)
Continuity mistake: When Tsubasa and Hyuga have their senseless head-to-head challenge, they have a low wall and a green hill and trees as background instead of the bleachers that were visible in the wider angles. (00:14:00)
The Verification Test - S1-E16
Other mistake: 15 players were picked for the team. Genzo is injured, 11 are playing. Three are MIA with no explanation. Morisaki has to parry 10 shots for every other player, he parries 100 total. Ryo Ishizaki though is still a substitute, so that would mean there's the unexplained absence of even one more team regular.
The Verification Test - S1-E16
Factual error: Judging by the x-ray of the leg, Genzo wouldn't even be walking without a cane, let alone play. His tibia is shattered. (00:07:20)
Continuity mistake: Under the rain, Sanae's hair turns wet in the space of a cut (dry when she's waving the flag, damp when she comments that they need just one more goal). (00:01:50)
Continuity mistake: Urabe is commenting on Musashi's number 9 being dangerous. There is a man behind Ishizaki, with glasses and a blue hat. He's shorter than the shortest in the kids' group, but later in a wider view he's quite tall. (00:15:15)
The Fight for the Final - S1-E24
Other mistake: At the end of the previous episode, Taki crossed for the corner, and the keeper punched the ball before Kisugi could head it. This episode shows that Taki crossed for Izawa instead.

Continuity mistake: Hyuga is looking towards Tsubasa; there'd distance between Urabe and him, but in the next shot they are arm to arm. (00:15:25)

Continuity mistake: In the first shot with Manabu cheering, he is the same height as his single-toothed friend. He is supposed to be shorter - just as he a couple of shots later. (00:03:40)
Other mistake: In the aerial view of the stadium that follows the intermission with the sushi chef and Genzo at his mansion, there are 23 players on the field. One is the referee, you might say. In that case, the view is missing the linesmen. (00:07:20)
The Youth National Championship - S1-E17
Continuity mistake: When the camera pans on the crowd for the first match, and we see for the first time Roberto Hongo and Tsubasa's mom, behind them there are 2 people wearing big glasses, and one guy behind Roberto is wearing a beige jacket. Closer angle, and just one person sports shades, plus Mr. Beige Jacket is wearing a powder blue suit. (00:20:25)
No Easy Way - S1-E12
Other mistake: The same play-by-play commentator covers both the Nankatsu and the Meika matches, happening simultaneously in two different areas of Japan (and with no television coverage...of course, who does live commentary on elementary school matches anyway?).
