Continuity mistake: Sharon Goodwin is having dinner with her ex husband to her right, and her son to her left at the table. As she is explaining her divorce to her son, he is looking to his left to address his father as if his father was sitting across from Sharon, instead of across from him. (00:38:10 - 00:39:25)
For the Want of a Nail - S6-E9
Continuity mistake: In a previous scene Dr. Choi had asked that a patient using a nasal oxygen cannula be given an oxygen mask, which the nurse did (over the cannula, but that is another mistake previously documented). Several scenes later we see that patient again, and he is back to wearing the nasal cannula and having breathing difficulties.
For the Want of a Nail - S6-E9
Factual error: Dr. Choi is seeing a patient. The patient is wearing a nasal cannula. Dr. Choi asks that the oxygen be upped and an oxygen mask be used. The nurse takes a mask and puts it on the patient. Right over the cannula.
What a Tangled Web We Weave - S6-E13
Character mistake: Dr Choi addresses April as Maggie. (00:22:51)
Suggested correction: He never uses either name. He says "hey" in a hushed voice.
What You Don't Know Can't Hurt You - S7-E12
Factual error: After an operating room nurse is unable to insert an intravenous catheter into a patient's arm during a surgery, Sharon Goodwin standing behind her volunteers to try. She inserts it flawlessly and states that she has placed "a 16 gauge in the AC." The catheter that she was holding has a green base, which is an 18 gauge. 16 gauge needles have a gray base. Factual error: Will Halstead arrives at Seattle Tacoma Airport, and when he steps outside of the terminal, the Space Needle is in the background. Unfortunately, the Space Needle can't be seen from outside of the terminal, since the airport is not in Seattle. (00:58:39)

Revealing mistake: Dr. Charles reads the patient's chart. Near the bottom, it says, "Is this enough filler? My hand hurts." (00:19:30)
What Happens in the Dark Always Comes to Light - S9-E3
Continuity mistake: A patient vomits in the face of Dr. Ahmad. The nurse wipes her face, but there is still vomit on the right side of her nose and under her eye. When she walks from the bedside, the remaining vomit is gone, although she never wiped her face. Also, the splatter on her shirt changes shape in the scene. (00:11:44)
