Other mistake: When Chazz gets out of the hospital bed, he appears to be fully dressed, including wearing a jacket, trousers and shoes. There would be no reason for him to be dressed head to toe in that moment. Even if the nurse didn't remove any of his clothes, it would be unrealistic for him to be wearing his jacket, his belt or his shoes while resting in bed, and would've probably been removed when he was admitted. Continuity mistake: At the end of the battle between Jaden Yuki and Gravekeeper's Chief, just as the explosion settles, in one shot you see the Gravekeeper's Spear card to the left of Gravekeeper's Chief on the duel disk. However in the next shot Gravekeeper's Spear is replaced with Magician of Black Chaos, which Chief neither has in his deck or ever played in the match. (00:13:35)
